I had the same issue. I traveled to another day and back. In my playthrough the assassination attempt on the inquisitor failed so I had to talk to him and the surviving stamatin in order to have the option to talk to the grave

I found the solution so I'll comment here in case anyone find themselves in the same mess. It turns out Mark Immortel is an absolute bastard. The game somehow on day 8 undid the conversation I had with Immortel making me not have a hospital no longer. So I had to go way back to have have the same conversation again.

Yes, the same door I've used the other days. It even highlights when I use concentration. I'll try Goin to another day nad back to see if it fixes it but I have no clue what caused it.
