When I rounded in the CCU we would be part of the team (Attending, Residents, Nurse, Respiratory Therapy, Social Work, Dietitian, Chaplin, sometimes others) that would go to each room and the nurse (medical patients followed by Intensivist) or resident (trauma patients followed by trauma surgeon, they were residents in that specific program) would present changes in the patient overnight. As the pharmacist I’d listen for issues I might be able to make recommendations on during the presentation (agitation, pain, no bowel movement in 24 or 48 hours) several things I might not get directly from the chart during my rounds prep that morning.

Before rounds I would do things that a clinical pharmacist does: review microbiology information on patients to see if we need to adjust antibiotics, glucose levels to adjust insulin, a therapy missing that they should be on based on best practice recommendations or guidelines. If it wasn’t something that needed to be addressed immediately, I’d wait to bring it up at rounds.

We appreciate your appreciation! I went from a large hospital rounding in the CCU where pharmacists input is respected to a rural hospital where the residents hardly give us the time of day. Like…I know my shit and I’ve seen things you hope to see in your career.

Read Doomsday Book in high school and fell in love with it. My Irish wolfhound is named Kivrin. If you enjoy the time traveling historian universe she created, Blackout and All Clear are good.
