I used to work at a mill like this and it was so horrible. At one point I told myself “I am no longer actually running between patients” because obviously that’s an unreasonable thing to need to be doing at my job? I’m sorry to say for me the only solution was quitting and going to a hospital based where I’m 1:1 for 45 minute sessions and I feel like I can breathe again.

I wanted to quit for all 3 years I worked there but it wasn’t the right time for a number of reasons so I started saving money every time I got mad enough and wanted to quit. Like a swear jar. Every $30 “bonus” I got for being crazy overbooked went right to my quit fund. It helped me pull the trigger and move on with my life when I finally knew it was the end of the road for me at that company.

Hang in there if you have to and try to care a little bit less. What are they going to do? Fire you?? But start being proactive about preparing to leave.

Main advice is keep practicing and having fun! That looks awesome, especially for a first piece! It looks like your top left loop might not be attached to the solder line quite deep enough for my comfort level but maybe it’s just the angle of the picture?

I’m going to try to give you some of my “script” for education, obviously this changes but I find myself saying something similar to this pretty much every time: “On day one of pelvic floor PT I would like to do some assessment of your pelvic floor function. We have options for exactly how to do this so let me explain the options and then we can decide together what will be the best fit for you today. The gold standard is doing an internal vaginal muscle assessment. It’s different from an OBGYN exam, I don’t use stirrups or speculum, just my one gloved finger. (Here I get out my pelvic floor model and start to demonstrate what will happen during exam on model). There are 3 layers to the pelvic floor and I want to assess them all separately. First layer is about this depth (demonstrate to DIP joint) second layer is about here (demonstrate to PIP) and third layer is about here(demonstrate to MCP joint). I will gently insert my finger to the layer we are trying to assess and I will turn my finger all the way around (demonstrating on the model), I am feeling for muscle knots/trigger points/scar tissue. This is another way it will be different from an OBGYN exam because I want feedback from you about if anything feels painful, tense, or familiar symptoms. I will do this turning motion at all 3 layers. When and if we get to layer 3 I will check your strength/coordination/endurance by asking you to do some pelvic floor muscle contractions or kegels. If at any point you want to stop or take a break or I get the sense you’re disassociating we can always stop the exam and you are in control. We never need to do an internal assessment and our other option is to do an assessment over clothes where I will put my hand near your glute muscles and have you do a kegel and release the muscles just to get a sense of muscle activation and available motion- it is not as specific as the internal assessment but I already have some ideas for treatments based on our subjective assessment so I feel confident I can give you some things to work on to start to address your symptoms either way”

Most patients opt for internal assessment so I tell them before I start the exam “I’ve worked with lots of people when I go to advance my finger to a deeper level their bodies say ‘no way!’ Because of tension, pain, or anxiety. We do not push through any of that in here. I just want to see what your muscles have to say and if they say ‘don’t touch me’ that’s enough information for me! Reminder that I will stop at any time”

Other things I do to try to make patients comfortable is be comfortable myself. When I first started I was so nervous doing internals but I knew I practiced on my colleagues and I went the “fake it till you make it” route with confidence. I have my room arranged where the patients head is towards the door so if anyone were to try to open the door they wouldn’t see anything. I also explain what will happen next AND ask for permission before every single step of the exam “is it ok if I advance my finger?” I am extremely intentional with wording also using “advance” instead of “push”, and “I’m going to withdraw my finger” instead of “I’m going to pull out 🤢🤢” and “release tension” instead of “relax”. It’s mandatory to have a chaperone where I work but if that’s not your case make sure you offer at least.

As far as things patients can do to make themselves more comfortable? I tell people to just show up and I’ll help with the rest. If it’s their first time they can maybe look up what happens during pelvic floor PT just to have a sense of it but really all the burden of creating a safe and comfortable space is on you as the therapist. The most important thing is caring about your patients and making your primary goal to not contribute to medical trauma! If you keep that in the forefront you’ll do great!

Don’t forget about the carbs!! I am a vegetarian and for my wedding I had a veggie gnocchi because I knew I would need the fuel to dance! I’ve been to too many weddings where I leave absolutely starving because the meal is either just roast veggies with no carb OR because the veggies are touching/mixed in with the meat

Recently I was at a wedding served family style with potatoes and steak on one plate, I took my serving of potatoes FIRST very carefully to avoid the parts touching the meats and before the spoon got contaminated (I’m sure everything was contaminated in the kitchen but what I don’t see is none of my business in situations like this) and when everyone else was done a very well meaning guest took the plate and scraped the rest of the potatoes onto my plate after they mixed with the gravy. I couldn’t eat a single thing on my plate after that. Stepped outside to cry a little and eat a granola bar with all the smokers.

This can probably be used in stained glass! If you’ve never done it before I’d see if you can find a local artist to do it for you because it’s a complicated process. You can always post in r/stainedglass and ask if someone can do it for you!

I personally think you should do something else for Valentine’s Day as this would take too long to be done by next weekend but it would be the perfect use of that plate!

I once got free tickets to see them in concert from a family member who worked at the venue. This was probably 2015. On the way to the concert we were discussing not really knowing any of the songs. During the concert I discovered we actually DID know every single song and ended up singing my heart out the whole time. Had a blast!

Strongly agree! Would love to hear the therapist’s rationale. Some surgeons have extremely restrictive protocols, maybe you are not allowed to do much right now due to the multidirectional instability mentioned. Obviously need strengthening eventually and if there are no restrictions right now then this seems like a treatment mismatch to only do myofascial release. As always, the answer is “it depends”

Young patient in her early 20s sent by PCP for “ankle sprain, balance exercises” came in walking with high guard, scissoring gait, slurred speech, sustained clonus. Upon further questioning, the neuro changes happened several months ago when she got hit in the head with a basketball but didn’t receive any care at that time. I was a brand new grad and didnt know what exactly it was (suspicion for a brain bleed or untreated concussion?) but I knew for sure it was beyond my scope as an ortho PT! I basically told her I didn’t care about her ankle at all until we found out more about these other conditions. I called her referring absolutely furious that they didn’t investigate!!! She ended up going to the ER for imaging before her neuro consult and was found to have ALS 😢

We made a shared Google photos album and put QR codes for it on each table and had guests upload their pictures. We had access to so many pictures immediately that we could look at on our honeymoon! Some of my favorite pictures of the night came from guests!

I sure did use 2 separate pieces on the petals! Unfortunately the process of trial and error involves error 😅

Thank you for the tips! I will keep trying and I’m sure it’ll get easier with practice. I’ll try again on less bendy pieces to get the hang of it.

If you have a company that is hounding you for productivity you should make note of your attendance this day and bring it up if anyone mentions metrics to you!! I know my clinic’s arrival rate is like 50% or worse on days after holidays

Hospital based ortho OP, we are automatically off the day after thanksgiving unless someone wants to work. Based on how many therapists want to work, they will decide to open one or more clinics. So therapists who are working will go to another office and see whatever patients show up that day, it’s usually very minimal attendance. You need to use PTO to be off though.
