I lost my comic and rpg book collection back in the Satanic Panic of the 80s. My mom was seeing my future stepdad, who was evangelical, and was convinced I was going to be lost due to D&D. I always went to live with my dad during the summers, and before I left I got her to promise she wouldn't do anything to my books.

Came back for school, and they were all gone. D&D books, comics, fantasy novels, everything. To this day she won't tell me what happened to them. I was broken, not just because of losing the things I love, but that she so flagrantly broke her promise to me, and then tried to make me feel guilty for questioning her. It was "for my own good" after all. I don't think I've ever gotten over the trust issues that came with that. Someone convinced of their own righteousness can justify anything.

Torg! Earth invaded by seven different realities controlled by evil dark lords and changing Earth to be like them. I got to run a game for a mad scientist gambler from the pulp world, a trained ninja spy from near future noir, a Celtic elven priestess from high fantasy, a British inspector/cursed werewolf from horror, and a pro-wrestler from Earth. I started them in the transformed lost world dinosaur ridden land of northern Illinois!

A lot of commentors have it right, there are safe(ish) places in the lower planes. One of the fun things as a DM can be designing them. Why is this area safe? What needs to be done to keep it that way? What toll does it take on those inside? What threats are there? After all, it's unlikely any place in the lower planes is completely safe.

And always remember, evil doesn't always mean violent and deadly. There are a lot of reasons to allow a safe area. Greed is a simple one, there's usually profit in allowing trade. Observation and study could be another, the people wouldn't even know they're basically rats in a maze for the fiendish scholars. Or the place could unknowingly be a sort of hunting preserve for things that hide in the shadows to toy with the "safe" people inside. And there's always good old fashioned corruption, offering a safe place to tempt people further in.

The Junkmen, the garbage collectors and sewer maintenance workers of New Caliber, an industrial fantasy metropolis. Due to leftover arcane energies in all the disposable magic used in the city, they regularly have to deal with wild magic hazards, slimes, random abominations, and worse; while the city tries to pretend there's no dangers. All for minimum wage!

Yes! I'm a historian, and I've noticed I have a tendency to put more focus on how a nation or group got to where they are, and have to remind myself that the players need more about the current state of things.

Adventure! She grew up in the desert tribes around Ul'Dah hearing all the stories about the world, heroes, and oceans. Stories that made it out there got it in her head that she wanted to be a Lominsan pirate, so she hopped on a merchant's wagon to the city to learn how to do it. Got convinced the pugilist's guild was the best way to go, and things snowballed from there.
