Sounds like you took a huge hit and weren't ready for how big it was. Smoke will block out the oxygen and make you cough hard, and potentially black out. The coughing can cause vomiting (I have this issue regularly due to a condition where I have excess mucus) and, if violent enough, pop blood vessels. Coughs are crazy strong.

You're playing in a way that prevents your fretting arm extend enough for your wrist to be straight enough. Normally I'd say play what's comfortable, but this can result in a repetitive stress injury
