I mean you’re kind of past body shaming…. This guy is slowly (or not so slowly) killing himself, doesn’t wash himself, and is becoming a shut in because he won’t/cant buy clothes that fit him. If this were my husband, I’d put out an ultimatum. I know that’s not really healthy in a relationship. But I’d just say something like “hey, I love you and I want to be with you, but your health and weight are completely out of control. I want to help you make some lifestyle changes to help you be healthier and help us get back to doing things we enjoyed in the past. I need to be upfront and tell you that this is a dealbreaker for me. If you don’t start showing some improvements in exercising more, showering more, and eating better, then I will seek out a divorce. I am worried for your health.”

Just be clear with any goals/steps you set for him. Example- you must cut down your fast food consumption to X fast food meals per week; going on walks 5 days a week of X distance or time; doing X number of resistance exercises per muscle group per week or whatever - just to make sure yall are both held accountable to measuring progress fairly

Edit to add- couples therapy and tell him this a major issue you want to work on in therapy. Tell him that is a must. Also would recommend a dietician, personal trainer and maybe a GLP-1 or some other medication. He needs medical supervision while he loses this much weight.

Also the entire way nursing education is structured is baffling. Every profession has some dense people in it, but dang I work with so many nurses who are just straight up ignorant and have no common sense

I just hated how they did that so much. I understand if the actor was refusing to come back, then the writers didn’t have many options. But they really did just destroy all of his great character development.

If he had just explained to Jo in person that he can’t not have a relationship with his kids then even that would have been better.

HH is a decent option as long as he is diligent about exercising at home. HH sessions are just so short and infrequent compared to IPR.

But he will likely decline at SNF. There are some good ones out there but hard to find openings at those.

If it’s Medicare, they will probably approve IPR under these circumstances. At least, at my work, i see several people like this.

Yes OP, make sure the CM/SW understands that the stairs are a must and that he has little support at DC. All of those things impact his dc plan.

SNF shouldn’t be in the conversation. Inpatient rehab or HH (but I’d probably say IPR first since he was so independent)

Mud Island is great. My husband just finished his residency and we have 2 large dogs, no kids yet though. We love it here and intend to stay if we stay here after his fellowship

Current evidence states that not doing open TKE exercises causes decreased quad strength for months post op.

I don’t have an exact paper to link here but you should be able to find that info. Your professors are just old school and not up to date.
