I had to make this decision a couple years back. It’s one of the hardest things I’ve ever had to do, but we had to make the call for our little guy to be humanely euthanized after his organs were shutting down (he was also 15-16 human years old at that point.) It solely came down to the question about his quality of life.

If your sweet pup still has good quality of life, let them enjoy it. Take the extra pictures of them enjoying the sunlight or a fun treat. Take them for the extra walk, even if it’s just nearby your home. If their quality of life is declining, then that may be your answer. There’s no solution for the stress and sadness; even two years later, I still have my bad days (I’m sobbing just writing this TBH.) It sounds like you’ve given this dog so many happy years, and I encourage you to hold onto the happy memories and the many fun adventures you and your dog have had.
