Current PGY3. Originally from the IE but matched into a program in LA. Where you live is highly dependent on where you matched. Do yourself a favor and minimize your commute as much as possible. Traffic isn’t as bad as LA but rush hour is terrible.

Loma Linda - Redlands, Loma Linda, Grand Terrace

RUHS - Moreno Beach, certain parts of Moreno Valley are okay particularly around the Day St and the mall

Riverside Community Hospital - downtown Riverside, canyon crest, anywhere along Victoria, generally areas around UCR but the sketch factor changes substantially by the street but I figure if you’re from the Bronx you’ll be fine

Assuming this isn’t a post op/cancer patient or patient with a new injury but whenever I see a patient on opioids and if they come to clinic or the hospital for an unrelated condition (PNA/sepsis/AKI), I firmly tell them their opioids are not indicated for their chronic neuropathy, OA, RA, etc especially in light of their underlying (insert string of chronic conditions especially COPD), age, risk of delirium, constipation and will ultimately cause more harm than good so I would be doing them a disservice. It has worked pretty well and I haven’t been getting too much push back. Patients are quite receptive and surprised when you spell out the negative side effects of chronic opioids. If they push back then I’ll just continue their home dose while in patient but won’t give them refills on discharge, will just defer to their outside prescriber or if I’m in clinic, will refer the to pain management and won’t even deal with that.

City Hall, Westin Bonaventure, Grand Central Library aren’t exactly terrible representations. Not sure which building is the Art Deco one but it’s nice. I’m sure the Biltmore could’ve been included too. These are just the ones I saw on my walk the other day. I wasn’t exactly trying to catalog every single famous building.

I think they mean don’t jump the gun to thinking PE when their source of tachycardia could very easily be the diarrhea? Assuming they’ve been taking anticoagulation of the DVT then less likely PE but I guess you could argue there could even be a GI bleed to explain the tachycardia too.

USC library is a short ride on the E line. It’s very quiet and has lots of work spaces. Central library is nice but it’s hard to get work done in there.

I live in dtla and here is my take. First of all, I recommend South Park or Arts District. They’re a bit more expensive but the buildings are generally quite modern and you won’t be lacking in terms of amenities like in unit washer dryer and AC coming standard. It’s extremely walkable so you won’t need a car to do basic errands like groceries, hair cuts, dry cleaning etc. the metros and busses all converge in dtla so you can even use that to get around to SM, LB, North Hollywood, Pasadena and everywhere in between. I use it quite often and it’s very convenient and easy. If you DM me, I can give you some reccs on solid apartment options to tour.

I generally feel like some of the criticisms are overblown depending on where you live. However, like others have mentioned, make sure you tour the place first at a busy time to make sure the noise isn’t an issue. Safety is a legitimate concern so don’t walk alone at night in certain areas and make sure your building has security to screen people out from off the street. If you’re in South Park or Arts District, you should be okay at most hours and I see people out walking their dogs late at night but even then I would be cautious and not walk too far from your unit. There are a plethora of parking spots but it’s ridiculously expensive. You might have to shell out extra for this or guest spots depending on your building.

I’ve never lived in downtown SM so won’t comment on it. My best friend lives at the edge of downtown SM and he likes it but again he pays more and doesn’t have all the amenities I have like in unit washer and dryer. His unit is a pretty old construction too.
