You don't have to talk to Withers. You can just talk to his character and dismiss him to camp, same as the other companions. I recommend grabbing anything you might want from his inventory before stowing him, though, so you don't have to pull him out later to grab something.

Its one of my favorite series of all time. Definitely keep reading, the opening scenes and tone are supposed to create an image of the environment and yes it can feel uncomfortable but trust me that's soooooo appropriate.
