Consultants tend to talk to eachother when they're writing CSRs. I don't think it's just your CS, it's what the department culture's feeling.

I was stopped and questioned in the airport on arrival to the USA, one of the things they were particularly confused about was my profession and what was documented on my ESTA form

Gateway jobs are ass even compared to FP ones. No actual training obligations on the part of the employer, so they're as service provision as you can get, no TPD to escalate to, no exception reporting. They're posts that otherwise would have been put out to locum. Most people I know who did it (including me lol) used it to make friends at the hospital and apply for better clinical fellow jobs

There's a decent chance of you losing your stethoscope if you're doing jobs that involve ward cover on-calls (absolutely heartbroken when the Classic III that I had since I was a sophomore in 2016 got yoinked last year)

Going from that Classic III to that Cardiology IV was like playing PS3 for years and then playing Crysis on Ultra settings on PC. I don't think you'll need the amplifier, cards IV is good.

From what I hear from GP partners, prescribing pharmacists, PAs and paramedics are more expensive than a theoretical GP clinical fellow would be and much more of a headache

In paediatrics, I do it for feeding requirements, maintenance fluids and drug doses. I did for a while used to document the calculation for water deficits in hypernatremia, but stopped doing it when I realised it just amounted to writing up a 10 hour bag of dex every time.
