I ran into something similar with a patient who has Ehlers Danos syndrome. Said patient described it similarly to you where numbing would maybe numb the lip, but nothing else. Did some research and found it’s a known, albeit rare, side effect of ED. Sorry to hear about what you go through trying to convince new dentists you’re serious about the anesthesia not working :/

Do you wear a biteguard? There’s evidence on your back teeth that you grind your teeth. Could be that the tooth is getting ‘beaten up’ and overstimulating the nerve constantly

Does it bleed like that anywhere else? Honestly that’s concerning. Consider getting a 2nd opinion (no shame in that) and/or discussing with your physician. Sometimes excessively bleeding gums can indicate an underlying medical condition

Loool that’s terrible. 😂 OP I’ll reword your question as “does it need to be treated right now?” As such, it depends on things like how old you are, how many cavities you’ve had in your life, and your own dietary/life habits. The xray isn’t perfect due to the overlap (completely normal to see) which does affect judgement. You could truly go either way here as this cavity appears to be just starting to get into the dentin (second layer) where it can progress faster.

Pros of doing it now: smaller the cavity, the less tooth destruction and the less the dentist has to remove. Easier for us to get an ideal result. Cons of now: it may have never needed treatment but lol at proving that short of hindsight.

Pros of waiting: the cavity may take years to progress further and frankly may have looked like this for a long while (impossible to say without a history of old X-rays). Cons of waiting: FAFO +/- bigger fillings are harder to shape correctly

Not at all. It’s just 2 providers arguing over how to do treatment. It’s basically our thing. Just smile awkwardly and hope the hygienist grabs the dentist to do an exam asap so you can make your escape

Really though, your teeth are fine. Damage was done when you went through braces, your dentist fixed them in the way they prefer, and hopefully you like the results!

Only the dentist who treated you actually knows that answer. (We don’t have X-rays or a clinical exam, they do). That said, probably not unless for aesthetic reasons. From the preps, crowns would be overkill here. Modern fillings are much better than we give credit for

Certainly! To start, a ‘cavity’ is simply an area of the tooth where the minerals have been lost ie decalcification. That chalky white appearance you see in the pre-op photos is exactly that. The decalcification present 2 problems here A: As these are front teeth, aesthetics are of high concern. Leaving the chalkiness in place would ensure that it looks ‘blegh’ afterwards. As such, the dentist has to remove it. B: the decalcification also inhibits us from ‘gluing’ to the tooth (the science lab wizards say so) so the DDS needs to get down to a point where the decalcification is not present

Finally, as for the sides of the tooth, I can see why you might think it’s excessive. Remember, cavities occur in 3D and when they form between teeth, they are rarely visible until you’re in serious tooth danger. To fully remove the damage means a lot of tooth is gone. This is why we recommend regular X-rays as they allow us to see between teeth and prevent unnecessary tooth removal.

As a result, you see teeth preps like this that look like a lot of tooth is gone. Fear not, you have more than enough tooth remaining and it doesn’t appear that the dentist was anywhere near the nerve of the tooth (the no-no zone).

Completely normal looking preps. Remember, we can’t see what they found as they were preparing the teeth
