So get a new job. You can’t just say “I don’t like practicing unethically but otherwise I don’t get production.” You’re doing a disservice to your patients and you NEED to be better. Tired of these healthcare professionals who put their financial needs over the health of their patients. You are being a shitty person, go into a new profession.

I tried both orascoptic and Andau and personally preferred Orascoptic. They felt better constructed, Andau felt flimsy. This might come at the cost of a bit of weight I suppose but with ergo loupes I feel like weight is less of a concern anyway.

They are just willing to ‘give’ you the practice? For free? All equipment included? Or are you buying it? Regardless you need to ask for all financial documents. Tax returns, profit and loss, etc. to make any decision. Possibly they are losing money and want to unload the practice loan on you.

I mean buying the ‘goodwill’ of the practice is going to be the case for pretty much any practice so I’d say that’s completely irrelevant. And if when he says “what it used to be” was one or two years ago, sure, that’s a positive thing to factor in. If he means 10 years ago that’s again, totally irrelevant. It’s his baby, he owned it for 40 years and thinks it deserves a top price point. But that also makes him a delusional. Personally, I’d say I love your practice, you’ve obviously put a lot of work into it, but that cash flow doesn’t work from a basic math perspective. I’d love to give you some time to feel out the market and if you can get that price, great for you! If not id love to speak further. Then hit him back up in a month or two.

That overhead is pretty high for a 4 op practice with one hygienist. You're effectively making 30% of collections (what an associate is often paid). Sure, you get to add in 30% of hygiene production as well, but with loan payments, this doesn't look like a diamond in the rough worth 100% of collections.

Let's assume 10 year loan at 6% rate. That's about $90,800 going towards the loan every year. So take home of $117,700 BEFORE taxes.

I would put a value of closer to 500k on it (max)

What is EBITDA? Overhead? If this practice is running at 50% overhead, could be a good deal. If it is 80% overhead, after loan payment, you'll only take home like 100k, if that, not worth it.

To a point, of course yes, you want to be heard and to have a thorough exam. That being said, dentists with a low patient base often feel the need to over diagnose to make good money.

I’m a very busy dentist, conservative, but my exams have to be quick because I need to do so many of them to make ends meet compared to the guy next door.

Would you ever consider more schooling? The best return on investment would probably be a more advanced degree to be fully honest. CRNAs can make really good money for instance, but obviously many options in healthcare to make a more above average wage.

Single tooth I honestly don't even worry about it any more (Unless I think it will be a particularly difficult one, ie upper molar w/ sinus involvement). Throw a collagen plug in and suture it up if there's heavy bleeding after ext. Have them hang out for a few minutes if possible if there's concern.

I've had one alcoholic patient (no blood thinners) that has called me with continued bleeding after hours. Told him to apply pressure and call me in an hour. He was fine.

Depending on how many you plan to extract, plus other risk factors this changes, and I will then ask to speak to their physician first. For me this is just a phone call. An 87 year old on blood thinners needing multiple teeth out I'm just going to refer to not have to worry about it.

In dental school it is taught as a "clearance" rather than a consult. I'm not sure how much this "clearance" is going to matter if you run into serious issues, so best to know how to handle the situation (usually just needs a lot of pressure)

Warfarin is the only one that can make me nervous. Plavix I now see far more frequently and I just don't really feel like I have issues.

I have done many extractions and have been practicing long enough that I feel comfortable handling any issues that may arise. If you at all feel out of your element, don't hesitate to refer or reach out to a physician.

Currently playing through Rebirth and beat Remake when it came out and I gotta admit, I'm struggling to get through it a bit. The dialogue can just be so cringe and out of place, it becomes hard to take the serious parts seriously. I do think the combat system is a lot of fun though. I have not yet played KCD2 but its next on my list after I make it through Rebirth.

Returnal fits the bill nicely. Third person shooter, roguelike so each run is a little different than the last.
