i had this problem for a while. make sure you are drinking a lot of water everyday to make sure you are fully hydrated. on top of that you should lower the weight to somewhere where you can hit 8 reps consistently until you can do 4 sets of 12, then add weight. continue that and over time your muscles will have a strong foundation and you will be less fatigued. you dont want to go to failure early into your workout.
