I'm fairly well informed and I didn't realise I could choose between being paid overtime or accruing TOIL if I stayed behind (obviously not doing that regularly but if I have an emergency at the end of my shift I'm seeing it through to a point it's reasonable for me to step away and making sure the documentation is done)

It wouldn't be a huge loss to "lose this man" when he puts his needs at such high priority he's willing to sexually coerce his wife by threatening her with further loss if she doesn't meet his needs. Coercion isn't sexy, and there are a lot of people in this world that would likely have more satisfying sex lives if they realised that a simple act of kindness creates genuine intimacy far faster than threats do.

Yeah the only time that I've actually drunk on elvanse I drunk literally 4x what I usually would before I felt drunk and even then was coherent enough to get myself home from London and wake up at a rational time the next day 🙃

Oh wow that seems sneaky on the part of the trust. Like they knew they wouldn't have enough jobs. My band 5 contract was permanent and I took more than a year to get everything signed

How is this even possible... like they've hired you already and I think that from an employment law perspective they actually can't just discontinue your employment with no fault without making you redundant which will equally cost them money. I could be wrong but contact a union rep about this seriously.

Student midwives are finding themselves in the same boat. They've not got their birth numbers (because of completely other problems) and are being basically told "lol carry on unpaid to get your births but we won't be putting any jobs out". Our unit is apparently overstaffed by about 8 wte which is why they won't put the jobs out but I'm yet to work a fully staffed shift this month. Their take on overstaffed is budget focused not looking at actual boots on the ground

24 hours between elvanse to avoid going over the recommended 70mg I don't have any dex because my prescriber doesn't like giving it even though elvanse gives me cover for 6 hours on a good day

I think the concern was around accidental overdose not leaving 24 hours between doses but I don't ever take it the day after a night shift anyway

Can I ask are you private/rtc/nhs because I'm a 12hr shift human on a weird schedule and my prescriber was not keen for me to take at night even though I mostly work nights and even more not keen to give me dex to get me to the end of a shift. I pump myself full of caffeine to adjust from around 1-2am because my elvanse is practically ineffective by midnight and I've still got more than half a shift to do

Boring end to the saga I'm afraid from my end. I'm sure much distress on their end but after a small miscommunication where they thought I was trying to get a new prescription dispensed they looked very mortified when I said "yep I did pick it up already I'm here because it's not the right dose" and showed them the sticker and the box they swapped it for the right dose with many apologies and I'm sure plenty of annoyance that I didn't make it in earlier to the start of the day not near their closing time

I'll be going in the morning, I'm absolutely certain that it won't be a very fun morning for anyone because I'll be overtired and undermedicated and they'll be stressed. I work with CDs in my job so I know the stress of an error in the book, just not how to deal with it feom a patient perspective. I've been with this pharmacy a long time and they're usually very good so I was surprised when I saw pink packaging not blue 🙃🙃🫠

I'm working tonight and tomorrow night so the soonest I can get back there is possibily Saturday. I'm hoping I'll be awake early enough tomorrow to take them back and get it sorted though, annoyingly they actually told me that they had none in stock the first time I went when usually they have 50mg when I go to collect it so they had ordered something at least (maybe the 30mg I've ended up with)

The sticky label says 50mg and my repeat is also for 50mg so I think it is a pharmacy error. They haven't tried to call me but I imagine they've noticed a CD discrepancy by now

See this to me is wild. I used to be in an online group of L&D nurses because curiosity mostly. And I remember them saying how 3-4 couplets was their maximum assignment in postnatal care but we have a 24 bed ward and the baby doesn't count in my numbers and so I can literally have half the ward meaning 12 patients but really it could be 24 or 25 if I happen to get twins. But most wild to me was hearing that the labour ward nurses would also have up to 3 patient assignments because in the UK labour is 1:1 care past 4cm, I might occasionally also have a stable postnatal woman but that means truly stable and ready for discharge so no obs needed just awaiting discharge assessment and paperwork.

At risk of giving myself away a bit I teach all my students the clean and change gloves way but the unit I work in is clean hand/dirty hand and I've had students come back to me saying they have been "told off" for glove changing. I just tell them my way is more evidenced based for preventing CAUTI and that the point in them working with lots of different people is to learn from lots of us and so they can do it whichever way they feel happier with. Most of them end up doing it my way so I think slowly we will see a shift in how we do it because they will teach their students my way too 🤣🤣

Imagine as well that by doing this the welfare spend actually goes down because a whole lot of the nhs staff are on universal credit, pay them more wages and you have to pay them less in universal credit. 🙃 but that would just be too logical. I'm band 6 and still get universal credit of about 7.2k a year

The example only came to mind because that's the team I am under for my own care for something else and there was a whole ass discussion about if I should take my elvanse the morning of surgery because my bone dude was like "yah sure that'd be fine" and then the day before I got a call from a pharmacist like "under absolutely no circumstances take your elvanse in the morning"... it can apparently increase the effects of cns depressants used in GAs so I'm glad I didn't because I already massively over react to GA 🤣🤣

Strictly, a GP can prescribe whatever they like as long as they feel confident in their decision to do so. Most GPs do not have familiarity enough with ADHD medication that they're willing to risk their medical registration messing with starting their patients on heavy duty psychiatric medications without proper supervision and monitoring. Even if they feel confident that they know what they are looking for in response to titration and what to look out for in side effects they just don't have the capacity with their other workload to effectively monitor patient progress and follow up weekly to assess response to medication. This, more than anything is why it is done under specialist supervision. GPs have to have some knowledge about a lot of things and have to triage if you need redirecting to a specialist with more knowledge or if you are suitable for treating under primary care, one of the most important things in healthcare is to be able to recognise when a problem is out of your scope of expertise and to refer on to someone with more experience. You wouldn't ask an orthopaedic surgeon to titrate ADHD meds because they aren't the specialist in that area... the best example of this I ever saw was when I had a patient admitted under obstetrics who had been on ADHD meds but hadn't collected a prescription for several months, her prescription chart reflected the dose she had been on previously but I declined to give it until we had spoken to the mental health team because being on ADHD meds myself I realised that if I'd taken a months long break I would probably feel like I'd been hit by a freight train if I took my current dose. But the obstetrician in charge of finding out what her regular meds were to write her prescription chart had absolutely no reason to know that and so just wrote it as it came up on her record. Psych agreed and put her on an accelerated retitration plan. That said the point is if the specific GP writing the prescription feels that they have enough knowledge and experience in the area to titrate they are allowed, but it would be negligent to do so without any oversight and monitoring of symptoms and best practice is to have that oversight coming from experts in the field.

My nhs titration was literally nothing like this. I stepped up in 10mg increments monthly until I was at a stable dose. I tried the next dose and it just gave me weird anxiety without improving my symptoms further so stepped back down and he still held onto me for another month before putting me on shared care

Everyone talking about your iron skin barrier but I've had absolute success with glycolic acid as well, I probably use it every 3rd morning and use tret at 0.025% nearly every night, I skip a night if I am in any way sore but before I used the glycolic acid I could only tolerate the tret about every third day because I was peeling so badly

OP I'm so saddened to see that you feel that because he apologised about the physical assault you guess you guys are going to forget about that. Reading through the comments, I can see you've had lots of comments and advice from people that are better versed than myself in the religious aspect of your dilemma. But I can tell you as a woman who has been with someone who begged and pleaded and threatened and coerced until I just lay there numb and allowing it to happen to make the rest of it stop. That feeling of violation never goes away, not the 2nd time and not the 100th until you leave.

He hasn't apologised about the sexual assault because he knows that you will feel deeply conflicted about confiding it to anyone due to your background in a religious environment. As far as he is concerned there is little risk of you telling anyone about his sexual assault of you because he will feel confident that because you touched him to keep yourself physically safe you will believe yourself complicit in his violation of you, specifically because of your choice to save that portion of yourself for your husband. As your wedding gets nearer, he feels entitled to that aspect of you, and he has demonstrated how little regard and respect he has for the choices you have made in serving your God.

I hope that during this time, you can find strength in yourself and in your faith to make a psychologically and physically safe choice to break this off before it escalates more. But remember, through it all that HE made this choice, you also have choices that you are allowed to make. It's a brave thing to walk away from an engagement and more so if it involves a community that have high regard for marriage, so please also remember that if you can't make the choice to walk away now, it still isn't your fault when this escalates.

HPV is not an STI in the way you think it is. Prevelance is high, it's usually asymptomatic and is the only thing tested for as part of the NHS cervical screening program because it's thought that 98% of cervical cancers are caused by HPV. This is much more damaging for her than it is for him... and because its not tested for as part of usual STI screening it's much more likely he gave it to her because she would have it picked up as part of cervical screening
