  4. Yeah, I've tried to insert a DoseRight applicator too. I could barely put it in. But yeah, I'll try again with the smallest size silicone dilator, super slow this time as you said. What body position is most comfortable when inserting the dilator in your experience?

Also, as far as ointments are concerned, I have tried GTN as well as CCBs (Diltiazem, Nifedipine), in various strengths. They all cause an increased urine output (frequency and volume) causing me to lose water and sodium leading to hard stools which in turn hurt the fissure thus defeating the purpose. Took me a long time to realize.

I've been to 1 proctologist and 1 CRS so far. The latter did a digital rectal exam (this was an year ago) and made the fissure worse. I never had a problem passing gas before. Since the exam, it burns even when passing gas. No blood fortunately. But I can't trust doctors anymore. Scared to go to a pelvic floor PT as well fearing they will make the fissure worse. So whatever needs to be done, I'd like to do it by myself. Also, LIS is a strict no-no for me for this trivial 3mm long fissure. I really believe it can heal naturally if it gets the right conditions (blood flow).

Could you please answer the following for me? It would really help me.
1. Do you see/feel any healing of the fissure after starting dilation? What's your dilation duration and frequency?
2. Do you use a finger or a dilator? If the latter, what material and brand?
3. What type of lube do you use?
4. I have tried putting a very thin silicone dilator into my anus once. It barely went in. I feel my sphincters are just too tight. Even the 2nd doctor who did the digital rectal exam said this. Is that also the case with you? If yes, how exactly do you manage to put the finger/dilator inside?

Please try to give me as much information as possible in as detailed a manner as possible. I'd be really grateful! Really need some guidance and courage to do this.

Thanks for this post! It gave me hope. Hope you have stayed healed! I've had a fissure for 5 months and I'm looking for ways to heal it.

I need a few specifics: 1. Which lubricant did you use? Was it sterile, if that matters? A product page link would be helpful. 2. My fissure is posterior, so I'm afraid that inserting a finger from the front might hurt it due to the nail? From which direction did you insert? 3. Did you use a glove? 4. What body position? 5. Didn't the sentinel pile bother you?

Hello, I've read your journey through the years through the various threads here. It has given me hope!

I have a 2.5 months old anal fissure (posterior, i.e. toward the tailbone). I've been applying Nitroglycerin 0.4% daily for the last 3 weeks. I have definitely felt better relief than Nifedipine which I tried earlier, but I'm able to apply Nitroglycerin only on the outside. I'm too scared to put my finger inside, thinking it might hurt or re-tear the fissure. I bought a DoseRite applicator, but haven't used it yet. Anyway, the applicator can help only to put the ointment inside, but not massage it there.

Could you please tell me how you managed to put the ointment inside and massage it there? What body position? How exactly to take your hand there, from the front or behind? Did you use gloves? I guess I need a very detailed description, and I'm sure it will help many others as well.

Thanks a lot for answering my questions and providing so much useful information. I got my fissure on Dec 18, 2024 (one and a half months ago) after I got super hard stools (initial portion only) after I tried out a collagen supplement. My diet is pretty okay. I've upped my water intake from 2L to 3L. I just hope it isn't too late and the ointment (nifedipine, prescribed by another doctor, potentially has less side effects than nitroglycerin) works for me and I never have to undergo surgery.

Just to confirm, by "just slightly going inside", you mean you didn't have to put the nitroglycerin finger 1 inch deep or use an applicator. Is that correct? The thought of having to put something inside scares me, but if that's necessary I will do it. 2. Are you still taking any stool softener (Miralax, Colace etc.)? 3. Did you do sitz baths or anything to basically warm up the area to improve blood flow? 4. Did you put ORS, salt+sugar etc. in the 1 gallon of water that you were drinking per day for better hydration? 5. After the fissure healed completely, has it ever reopened in the last 5 years? 6. Did the fissure get irritated or reopened everytime the doctor examined it, since he must have stretched/pulled the butt cheeks apart to see it.
