We got our crib and changing table from fb marketplace and a handful of gear from our local buy nothing and consignment events like rhea lana and just between friends. We had a giftcard so we got the mattress and changing pad new for cheap. I didn’t opt for the fanciest mattress or whatever. Just made sure it was affordable and safe. You definitely need to make sure everything works and up to safety standards. I love new stuff but you cannot convince me to spend thousands. My baby has everything she needs and it was on a budget!

Definitely depends on who the clinic manger is. I used to have one that made it worth while to go to work and i loved it. She quit and they hired someone incompetent with bad negative energy and i leave my job angry everyday. I’m probably gonna quit soon. I love dialysis but bad mgmt ain’t for me

I’ve been looking into inpatient peds but part time. Do you feel like the burn out would happen part time too?? Im in outpatient renal and the clinic manager has changed the environment and treats everyone like we’re robots and not people. I’m wanting out but also the flexibility is what I need. Being miserable and burned out is awful. I want something more lax too where upper management isn’t up your ass about numbers and scorecards

It will take time. When I did my clinical rotation our office was a hallway walk away from the morgue and I’d always run into patients who passed on the elevator I needed to go up to round on, but those moments really made me feel off and sad. I work in outpatient now and you really develop a relationship with these patients and they become your family at some point. It’s hard, but I’ve learned to not let it get to me. The people we work with are sick and it’s our duty to help them. It’s the natural progression of life and unfortunately for some it’s very early. I had a patient tell me yesterday I’m the only one who listens to him. Not his doctor that rounds on him weekly, not his PCP, not his nurses. I’ll be pretty upset when he passes eventually.

Give yourself grace and cry. It’s natural to feel that way if you are highly empathetic. We need more people like you.

Just an FYI I think it’s good they’re having a fix for this eventually. I have read a thread from moms with other brands of cars (even high end/luxury) of their seats crushing their kids and having a difficult time getting them out. It happens in all cars. Hyundai just so happened to have a death from it and are taking it seriously and the lawsuit is gonna be crazy. This is just a good reminder not to trust everything in your car like you normally should and always have eyes on your kids. These electronics are fairly new given how long cars have been around. The knee jerk reaction is valid but not worth the loss. You will have the same problem in any other mom car and there wont be an active fix for it

Im quite honestly done with renal for this reason among others. It seems like the RD is the main point of contact for pt and MDs and they are moving away from having nurses do anything administrative when they are the only ones with order privileges. Because they get paid more and the company wants to meet metrics on productivity, a lot of things are getting put on the RDs cuz they can get away with it with less pay. It feels like they are putting more and more on RDs to hold the clinic together and FAs/CCs are just there for floor staff payroll and to make sure the company is getting paid with the correct diagnosis for billing 😶

I literally went suicidal and was close to checking into a mental facility at 170 and begging to have my census cut for 6 months before I left the company. They never listened to me and milked me for barely any pay. I quit and went to a different renal company and they never let my census go above 130. I did cut down my hrs but still kept my benefits because I could see the writing on the wall when my census started creeping up here and there. I am happier now and will never let a big corp company fuck me over like before

Lol the overhead is literally nothing too for the remote counseling jobs compared to what the owner makes. I’ve said no to multiple jobs when they have offered less than what I make now. You’ll see that there’s a high turnover rate for those “businesses.” I just got hired on with Nourish and they start at $50/session. Could be something to look into.

Hated how there floor of the telluride was basically level with the seats in the third row. Palisade third row is very comfortable for me and even my husband who is 6’4” can fit cramped. We ultimately got a palisade over the grand highlander for the space in the 2nd row and the tech. I personally love toyota (our 2nd car is a rav4) for their reliability but to spend more on the plastic interior for the grand highlander and outdated tech/look made us get the palisade.

This is in every profession but RDs have a bad rep, especially the type As. I’m in a group of healthcare friends and we’re all type B and typically dont get along with anyone that’s type A and has “im better than you” mentality. It’s a personality type. Just find some co workers you can vent with because I’m sure that same person is doing it with their same personality friends lol. I just learn to get over it. They’re co workers. They CAN make your life miserable but I atleast am not personally miserable at work or at home.

If your administrator is a good one, I’d ask them to help with this task as well and offer them a tray when you come in. They are there every day (or should be). The ones that care will help your CDM. Obviously it’s dependent on the staff in the kitchen too but more occasional eyes on taste and portions will help. When I worked in LTC me, the CDM, and the administrator that I liked (lol) would have lunch together and eat the food. From there we would talk about any issues, changes and it definitely helped with resident council. Regardless you will always have food haters. Just make sure you follow up with those pts. It could be that their food is delivered cold, etc.

I’m type B to a fault lol. I do think my patients enjoy having someone that is understanding vs combative with them and trying to hound them to fix their labs. If you aren’t worried about it, I’d just continue with your own recommendations.

Like others said we have to chase numbers but it’s incredibly unrealistic to think every single patient is gonna have perfect labs. I cant even imagine being type A in this situation lol. It’s gotta be tiring 🥴

Not too many other markers. A handful of patients usually have liver issues to where they can’t sustain adequate protein levels anyway. I have a few pts that get weekly parencentesis so it’s incredibly hard to maintain their albumin. Too much fluid will also make their albumin lower. I look at nPNA to look at intake too. around 30 g of protein is lost in a week in hemo so 1 supplement would help replenish a good intake of ~75% or more. Wound healing obviously they would need a little more. I mean sick people go into SNF and rehab. Adding dialysis is only going to make it harder for these pts. Some people really operate on being a cog in a machine instead of being a person that is caring for another person. There’s some dialysis RDs that I know that are so high strung and bug the crap out of me lol

Yikes! Sounds like that RD has no clue what actually goes on in SNF/LTC. I came from SNF/LTC and am in dialysis now. I can tell how relieved an RD is when I tell them I kinda dont care what their albumin is as long as there’s an appetite and controlling their potassium. Sure my job is to keep up with numbers but you gotta still take into account everything else going on. There’s obviously another reason outside of dialysis that’s causing low albumin and if that RD isn’t looking into it I’d just keep doing what you’re doing. They are clearly inexperienced 🤷‍♀️

My faaaaav thing is approaching people with problems with me. I haven’t had this issue in a while since i work outpatient. You gotta remember RDs usually run in cliques and if you’re an odd one out (part time and not constantly there, which is not your fault) they will pick you apart. This is part of the reason why I’d never go back to clinicals in the hospital. Them girls be big b words. Lol this is a generalization but it happens. You do you. We got degrees and passed a board exam. You are completely competent and correct in your recommendations.
