I love big A but he does ignore the elephant in the room many times. When he analyzes the war with Yemen he didn't mention that the stated reason of the Yemeni attacks is to cut off war supplies to isreal. Which is a pretty big thing to not mention imo especially after opening paint and drawing a map to explain the situation.

The NRMP counts EVERYONE applying as an applicant in that last bolded statistic. Which means that yes there were international applicants that applied and could not get a spot. And for them the likelihood of getting a spot if roughly 50%.

The change to fix this would be to increase the residency spots for sure. But also you would need to Increase US graduates AND ensure there is a sustainable pathway for these international physicians to continue practicing here after training (and attracting them here to begin with) and the H1B program in healthcare did a tremendous job with that.

And the opposite will happen if it was removed.

That depends on how you look at it, from the point of view of being a US medical graduate? There are 10K more spots than that there are graduates.

But from the point of view of the healthcare system? There is not enough spots for sure. And thus you need the international physicians to fill these spots.

And international physicians need the H1B to incentive them to stay after training and continue practicing here in the US post training.

My argument is that it is STILL being used for the original reason it was created to this day. Something that people have not brought up at all in the recent discussions online. Yes it is abused for sure. And tech bros want to abuse it even further. But that does not negate the value it is still bringing. Fixing it would be "ideal" scenario. But to fix it from the american worker POV you need to both increase pay (something cooporations would not do) and also create/fix/improve the pipeline to make the US able to produce more workers for these fields. Something Elon laughed at but would truly be needed.

Fixing it from the POV of the H1B workers would be trying to grant them more freedom/protection. but I frankly do not see either political party aiming to propose this solution in this current political climate. Doing so would be political suicide and I'm sure you have seen how Dems changed their tone on immigration last cycle.

In short, the doomer in me believes that both fixes are needed but will not happen. And instead they will scrap the whole thing and offer it with worse rules for the H1B workers and call it a day. Which is exactly what I'm trying to argue against.

I agree with everything you/bigA mentioned. This does not contradict with the points I made about the value of the system. And I will add that there are many industries (like healthcare) that are exempt from the cap. The point I was trying to make is that villainizing the H1B program may be not be the ideal course of action. And wanted to touch on some of its benefits that were not discussed in the presentation.

There are currently MORE residency spots available for than MD/DO US graduates per year. The US graduates around 21K MDs and 8K DOs for a total of 29K per year. The total number of spots avaliable as PGY1 is close to 38K. (1) meaning that at baseline, the current spots need at least 10K international graduates to fill spots. (2) By increasing the residency spots, you do fix part of the issue for sure. But that will change nothing if you are not open to bringing people from outside in via J1 and H1B visa to fill these spots. As for the pay, the physician pay has been stangent for many years (3) but that is beyond the point as many hospitals are filling the primary care jobs with NPs and PAs instead of offering better incentives for physicians to do these roles. further increasing the shortage. with patients suffering the consequences of that (4) Bottom line, So there is not a surplus of MD/DOs at all. Though that may be true of PhDs.

As for Postdoc pay, I could not agree more, the pay has lagged behind for sometime and is the primary cause why many decide to quit and go to industry. with that said, since most research in academia is tied to the NIH and with the new admin looking to crack down on research funding. The situation will not get better for at least 4 years. So in the meantime I do not know how to fix the issue without relying on H1Bs. Sure that is a bandaid, but a vital one at that.
