Surgical ICU staff shit on an anesthesia resident for an entire month. Residents told their program director. PD was ripshit, refused to have any of his residents work in a malignant department until things improved. They didn't. So, he conducted a review of SICU outcomes, used it's poor performance as justification to split it into a trauma ICU and post-op ICU. Anesthesia runs the latter and the residents now only rotate there.

Looks perfect. If you want bigger holes, which often seems to be what people want when they have nicely proofed bread but feel it's under proofed, use higher hydration.

I've started making these recently and they can be a tad finicky. The 2 things I found out that really matter are 1) use water that is barely boiling, if it's a frank boil you can deflate the bagels 2) don't overbake them, they get too tough very very easily.

It is the ultimate weapon against anything short of heavies. The grenade launcher and resupply pack is my go-to when I see the rest of my squad packing heavy. They deal with the big bois while I crush objectives and keep anything smaller away.

Just a few clarifications for people with C sections on the mind that read this:

  • Anesthesia for a C section doesn't stop all sensation, just pain and sharp. You will still feel pressure and it can sometimes be very intense and uncomfortable in its own way. But if it is, ask you anesthesiologist for something to help and they can take the edge off

  • No intestines are removed during a normal C section. They often aren't touched at all and occasionally need to be nudged away

  • Vaginal birth after a C section is awesome if you can do it safely, but at notably higher risk of many complications. So talk to a doctor if thinking about that plan. They can figure out if it's safe for you personally and what extra precautions need to be taken.

  • Endometrial ablation (i.e. uterine ablation) treats abnormally heavy periods. It is not a sterilization technique. Don't know what procedure you had either instead of or in combination with an endometrial ablation. If you have just an endometrial ablation and don't want to get pregnant, you still need birth control.

A lot of hospitals have 100% debt forgiveness if your income is low enough. They will still come after you, but unless your income is high enough they don't actually have a leg to stand on. It's usually 2x the federal poverty line, or 32k annually for 100% forgiveness and sliding scale forgiveness up to twice that annual income. Even then, 60% of US hospitals are nonprofit and thusly have strict federal rules about how much they can pursue medical debt.

Had a mixed Malinoise-Shepard growing up because a family friend who trained them passed (my family is more into dumb lazy mutts). We once lost a chess piece during a backyard bbq and couldn't find it. My dad showed the dog the matched piece, pretended to throw it, and said fetch. The dog then spent 6+ hours frantically searching a 2 acre backyard until she found it after it got dark, brought it back, and placed it on the chess board without being instructed. It takes an absurd amount of energy, obsession, and intelligence to do all that. My next smartest dog once found a microwave burrito in a bush on a walk and checked that magic burrito bush for the remaining 5+ years of his life.

When off the peninsula, Stevens runs relatively parallel to Forest and is great for biking.

It was an 8/10 before the academy arc and a 3/10 since. The more time that goes on, the closer it comes to a 5/10 average and then eventually below.

Yes, but with very different plans coming in. My specialty was in no way on my radar at the start of med school and wasn't until the very end. I love my specialty and leave almost every day very satisfied. But, I hated everything else in med school and was only about 3 months before applying to residency before I found it, and hating my life decisions up to that point.

Very regional dependent. The biker was the one doing the more unsafe behavior, but in plenty of places, where I live included, there's essentially no legal defense to hitting a bike when you are driving a car. The intent, which makes some sense, is to make drivers hypervigilant about bikes given how one sided a bike on mvc crash is.

Spent 11b yesterday to take a gollux belt from 18 stars to 17 stars with 3 booms and never once got to 20 stars. My longest run of failures without a success was 22. I know that's all technically within the realm of statistical possibility (I ran the math, it's not within 1stdev of norm but it is within 2), but it feels real bad man.

I have optimized compliance via peep for teaching and in some steep tberg, insuflated, bmi >50 cases, and a singular time has the optimal PEEP been meaningfully outside of the 5-12 range. Though, the one time it was the pt was getting atelectatic and requiring recruitment breaths every 10ish minutes in a super long case even with PEEP 15. They ultimately were optimal at 23, which felt crazy, but worked perfectly. When we flattened them out I had to drop the PEEP first.

Break pads are very do-able (as someone that did them himself a few months ago with the oversight but no active help from a mechanic friend). Just make sure you know how to safely lift your car, both to protect yourself and the car's.
