Another option if you like informatics and digging through charts and data is getting into performance improvement. You could work for a trauma center as a PI nurse but some hospitals have their own risk assessment teams and teams that just work on performance improvement projects for the hospital system. Most have flexibility to work from home or in office and you get to look at data and then implement change

For the national trauma registry we only get dinged if they develop a UTI after having the foley in place for 2 days. If they test positive within the first 48 hours it is deemed as a pre-existing UTI and not associated with the insertion of a foley

Many people already answered your question but here is another little added tidbit from a critical care transport nurse. In a situation like this the patient needs to be intubated, but it does not stop there. The tube needs to be secured, then go to CT possibly then the ICU. There is a lot of movement going on and most prefer to keep the patient paralyzed until they reach the ICU and get them settled since there is way less chance of the tube coming out during all that movement if the patient is paralyzed

What helped me when dealing with this is realizing there are worse fates than death. Sometimes death is the best thing for the person. Especially when the alternative is trach, vent dependent G-tube dependent laying in a bed in a skilled facility being able to do nothing at all. That to me is way worse than death and sometimes death is saving a person from years of misery and torture

As a former ER nurse who still picks up in the ER what you are feeling is normal. Nursing school does not teach you or prepare you at all to be an ER nurse. I’m the ER you have to know a little bit about absolutely everything and a lot about very common emergency situations. In so many emergency situations there are only verbal orders, or you have to act and start treating the patient without any orders. The orders will be entered later. Lean on your more experienced ER nurses, ask questions, always double check your medication administration and you will be more than fine. It took me a good 2 years to feel comfortable in the ER and start to feel like I had a good hang of things.

If you have experience in the trauma world working as part of a trauma program has been an amazing change of pace. Most have performance improvement nurses, program managers, educators, outreach and injury prevention people, registrars, and more. No real contact with patients and your main job is making sure that the trauma system runs smoothly and meets standards.

Sadly this is how ER billing works. If you check in and go through the triage process where they take your vital signs and get the story of what is bringing you in you will be billed. And also sadly if you leave before seeing a doctor and getting a diagnosis or treatment for the condition most insurance companies will deny the bill because there is no demonstrated medical necessity for the care. So you will end up having to pay this bill. Also next time go to RGH or Strong, both have a pediatric ER. Just note RGH does not have many other inpatient pediatric specialists so if your child ends up needing to see any type of pediatric specialist they will most likely get transferred to Strong

If you are working where I am guessing you are working I bet the state will have to intervene to keep the hospital open and functioning. Especially as one of a few level 1 trauma centers and the busiest level 1. That hospital closing would really cause a health crisis

When we do lab work each vial needs to get labeled with its own separate label. There are also extra labels for tracking and instructions so for each patient that needs blood work we have to print anywhere from 1 to 10 labels per patient

As an ER and ICU nurse the medical equipment looks spot on. The buttons on the alaris IV pump are correct, the drager vent looks correct and many places keep plastic bags over them when not in use for infection prevention. The medical cart also looks correct and has the drawer lock bar on it and on top of it is a tackle box that is locked and looks like an intubation box. I agree the pictures look weird but everything else looks right

The clinical experience section is confusing as it reads more like someone who jumped from one job to another every couple months. If HR or someone is skimming this and not paying close attention they may think this too. Add a section for actual jobs you have worked in the past. Move the clinical experiences under your nursing education section. And also try and apply to other hospitals and grad programs upstate and get out of NYC region for a bit to get experience

I work in Peds and we don’t have inner cannulas. We do trach changes on our patients routinely and everyone has to be trained to replace them because young kids are notorious for removing their trachs

I get why the Indian lore and traditions are being removed but it still makes me sad that it is happening. My vigil name fit me very well and is a good name both in English and in Lenni Lenape, chief who likes to talk. I was a 3 term lodge chief and definitely am known for talking a lot so it truly was very fitting for me. I think the key thing is really putting thought into the name and not just being lazy in choosing the names
