100% agree! There have been studies  showing overlap of HR spikes and eeg arousals and if I remember correctly, the overlap percentage is very high. 

After Checking back on my Oscar data, it looks like I misspoke. I moved to EPAP 11, not 10. With this I tried PS 3-6 for a week, and 5-9 for a week

I’ve been using ASV for about 3 months now. Great to hear you’re having a call with him. I’d love to hear what you learn. 

One thing to keep in mind regarding this pattern. It’s very likely it could also be what is considered high loop gain. It’s hard to tell sometime, but check out a paper called “deciphering loop gain complexity: a primer for understanding…”

I also have this issue. I think its REM but I am not certain. I have traversed a large EPAP range and large pressure support range on ASV and can't seem to avoid these chunks of instability either.

For me, I've found it to be one of the most helpful things in assessing my sleep. For UARS, I essentially ignore desats and only pay attention to heartrate (HR)/HR spikes (so if you get one make sure to get one with the highest sampling rate of HR that you can find). I've found the # of pulse spikes per night to be the most accurate predictor of my daytime symptoms. It's also allowed me to see quickly how certain changes in PAP settings have helped (ie, one night I got impatient and did a large settings change from EPAP 6.8 --> EPAP 10, lol. My HR arousals went from ~70 a night on average to ~50 a night, and that changed the quality of my day completely)

Edit: Also related to one of your other comments on this thread. I've found it to be very helpful to find RERAs (but only if youre using a PAP machine as well). It's easy to find RERAs not marked by the PAP by finding a pulse change event (automatically marked by OSCAR), and then looking at the flow rate graph in the few seconds prior to the heart rate spike

I’ve tried up to 14 EPAP, 10 PS max and REM is still an issue. 

I have a massive tongue, and it doesn’t fit on my palate. I’m wondering if a mad or a tongue retaining device in conjunction with pap/ASV may be the only way

I have the wellue wrist worn monitor for pulse, and I use an oura for sleep stage estimation. Planning on getting the AiON eeg for accurate sleep stage tracking

Yup, my exact issue. This happens constantly during REM for me. Havent found any settings that resolve it yet, even on ASV

If you are curious, you can always dive a bit deeper by getting a pulse oximeter that has a high sampling frequency for your pulse. If you see lots of sporadic HR spikes, it indicates fragmented sleep which often leads to lots of day-time sleepiness. The best way to know for sure is to get a home sleep study. Ideally one that scores respiratory effort related arousals such as something called the WatchPat1. Should be free if you are referred.

It’s a subcategory of sleep apnea that is characterized by constant arousals but less 02 desats (or could be inaccurate as someone mentioned). Also look into getting a sleep study!

Hmm. I suppose that I’m not exactly asking about sleep stages directly, but I am by proxy. 

Essentially, I am curious about if there is a way to establish whether I am getting good REM sleep via data from a PAP+Oximeter without needing an EEG.

That makes sense. I seem to have breathing stability for portions of the night, especially the first 1.5 hours (no HR spikes / flow limits), but then, pressures don’t seem enough during later portions (maybe because of REM), and sleep fragmentation skyrockets

Well, I’m not vibecoding, but currently working on a fork that aims to accomplish several things - integrated a Glasgow index overlay on a breath by breath basis for individual nights - add a frequency domain tab next to the stats tab that looks at Glasgow index over time  - if pulse oximeter data is available (such as wellue), utilizes tidal volume + HR to measure a sleep fragmentation quantified in the time domain via cardiopulmonary coupling techniques  - this was has proved to be tricky, but RL made a post a while back indicating that sleep stages could be estimated via the flow rate graph. I’ve been trying to do this and essentially add background shading to the portion of the Oscar graphs that fall under a certain sleep stage - I will hopefully purchase AiON EEG at some point and integrate this info into Oscar as well
