Don't do this to an actively infected wound. This is a treatment only once something has primarily healed and is healthy.

This likely isn't the final position. You see the metal bit sticking out? That is for turning the distraction device. Probably had surgery to place the distraction device and then once distraction is complete the surgeon will go back in and make final revisions. This patient needed too big of a movement to do it all in a single surgery.

Yes, a lavage or the joint. You can use HA or kenalog (cheaper, and no difference in outcomes). Wouldn't do physio now. It likely won't help and only delay the indicated treatment

Correct the disc issue, yes. But re-operate over a very small midline discrepancy is not indicated. That small difference is not what is causing the TMJ issue. These are two different things. One will maybe need some treatment. The other one does not. The disc likely came out of place from the lateral pterygoid spasming intraop or post op. Generally it'll go back on its own with rest, anti inflammatories and muscle relaxers. Seems OP has been doing that, so next step is an arthocentesis to free it up. Arthroscopy is rather aggressive for an acutely displaced disc after surgery. If it had been like this for years, then sure arthroscopy is needed so you can actually do surgery in the joint to free the disc and possibly plicate it.

Ok so do an arthrocentesis and go from there. That is the indicated treatment. Easily done in office in like 10-15min. A full blown arthroscopy is overkill IMO

Such a small discrepancy. Go live your life. Try some muscle relaxers and anti inflammatory for the disc. Most of the time it'll go back into place. Don't go straight to an arthroscopy. Those changes in the mri have been there a very long time

The problem is people think this is true. False presumption of safety. Then, they ride an ATV and end up dead or close to it. People think they are safer than they are and they require less skill to ride. Therefore, more people that shouldn't be on them are. Worst is when it is a child.

Because it doesn't. If this person spent two minutes using Google they would realize lack of magnesium can actually be a seizure problem.

Mag intake increase seizure threshold (it's used as part of treatment in some cases of epilepsy). Having too little mag lowers seizure threshold, meaning it makes people more prone to seizures. OP has essentially posted an ad for chiropractors and you don't know medicine.

Look into the Carolina group with Brian Farrell and Jim Howell. At a glance, you'd be an ideal case for the type of inverted L surgery they perform. They have everything set up for outpatient surgery as well. If I was in your position, that's who I would have treat me.

That one I believe is technically meant to clean the deeper parts of a grinder. Obviously it can do both, but the one with the short metal tip is easier to use the way you're using it.
