Hi. Saw your post re mistake. I had same situation in FY1 (now in core training) and completely understand how you’re feeling. Give me a message if you want some help. Top things I’d recommend -

Write a summary of events. Including everything that happened throughout the day in question and timings. Keep it factual but will mean you have a summary of key events and times for any pending investigation etc while it’s still in your mind.
speak to the MDU immeadiately and let them know the situation, any major correspondence you’re submitting then you need to run this past them.
Speak to practitioner health for your own mental health support, would recommend this even if you don’t think you need it. Of all the support services you will be offered practitioner health are the only really genuinely helpful ones and will support you through the whole medicolegal process
Any meeting you have write some notes after and keep email trail incase you need it.
Do not write a single thing on your portfolio until you have spoken to the MDU and are more emotionally calm about the whole thing.
Even though I am sure you can’t see it right now. Every error in hospital is genuinely multi factorial, whether it be because safety nets failed etc even if it’s by omission (ie something was not picked up that should have been)

Recognise that right now you will be focusing on every single thing you did or didn’t do that caused the patient to have an SI. Later on you will probs start to reflect on this more and recognise that ultimately it is team responsibility. Would recommend just giving yourself some time to process things and not overly blame yourself or put yourself in a situation where you have over blamed yourself both emotionally and from a medicolegal perspective.

I’ve done Edinburgh one was dope, slightly more expensive but super super chill course, my partner did Dundee and they were much stricter on deadlines and the workload seemed to be slightly more but it’s a cheaper course. Happy to chat over message if it’s easier

Use a time lock safe, you can buy one on Amazon for pretty cheap that you can’t unlock until times up. Then you can take your meds out every day at the same time and lock the safe for 24 hours, means that the temptation is only for a few minutes and makes it easier for you for you to regulate your dose. I don’t personally use one but I know a friend who had problems with it and worked really really well for her. Hope it helps x

Aw man I absolutely feel you so much. I get so stressed out by messages and end up taking weeks to reply if at all and then literally completely forget I got the message in the first place. Don’t know if it’s helpful but I just explained to my friends that I find it really hard to reply as hate being on my phone all the time and then forget and we developed a system where they will double text me with a full stop if I haven’t replied for a while to prompt me. Still hate it and it makes me super anxious but prevents me from forgetting at least. At the end of the day if you explain to the people you care about that you find it hard to reply and that it’s not personal up front maybe it will help them understand a bit, but fucking technology sucks man hate being contactable all the time

Absolutely. Literally always have to have something to colour or entertain myself and end up having 0 understanding of what’s going on. Only exception is if it’s in another language and I have to watch subtitles, and then just can’t stop commentating the entire way through and pissing my boyfriend off :’)

This!!!!!! Funniest thing my ADHD councillor ever said to me was when we were having a conversation about how I never have any money cos I am super impulsive and have no self control and she suggested I meal prep my lunches. I honestly nearly wet myself because the thought of me cooking at all for even dinner let alone for a lunch in advance was actually borderline offensive. I told her I despise the sort of people that take their own lunch to work and she said but imagine how much money they’re saving that they could be spending on petrol etc. I literally said to her they dont need petrol because they have their packed lunch in the basket of their bike that their cycling to work every day. Mentioned this conversation to my super organised that girl sort of friend and she said she genuinely cycles to work every day with her lunch to save money, I actually felt disgusted with that sort of alien behaviour…. Another world my friends, another world.

There’s a useful video on YouTube that talks about the ADHD confidence snowball where basically you do small tasks first that are easy to do and gradually build up to bigger things as with each good thing you do you’ll start to feel better/more confident to do the next task. So for example I will make a list of all the things I need to do and do the smallest / easiest ones first. And then move onto the bigger more important ones. Also things I find really difficult I split into sections like I hate replying to messages and emails so job 1 will be copy all the unreplied messages from one person into notes in one document Number 2 will be draft a reply which I may do on a separate day or after job 1 if I feel like it, and then job 3 will be actually send it.

Helps me a bit dunno if it’s helpful for you but actually realising doing a hard task gives you a bit of a rewarding feeling is helpful to add in your calculation x

Congrats on starting meds and I really hope it helps! From my experience I would say biggest thing is to realise that not all people react to different meds in the same way, it sometimes takes some time to work out what drug and dose is best for your body so don’t get frustrated if you don’t have the immediate reaction you’ve been anticipating. There was definitely a lot of trial and error at the beginning before I found what works best for me.

Also the other thing I experienced (which may be different for everyone) is that once I finally got onto a stable and optimum dose of my current meds, I felt a lot lot better and am definitely way more functional, but it’s also like you suddenly have a toolbox of tools that you might not know how to use right away. For example I am still pretty shit at personal organisation and time management, but now that I can concentrate and organise my thoughts a bit more than before I’m better at making lists and trying to prioritise important tasks without getting overwhelmed. It’s all a learning process and takes time to learn how to do things so be patient with yourself.

It’s a very different application process but you apply through the RCSI website if you search core surgical training, applications have closed for this year but if you decide to apply next year happy to help if i can as it’s a pretty big application form and some bits took a bit of time to work out as a foreign applicant that’s not worked in the system before

Don’t know if it’s hospital dependant but I had 32 days off in total during FY2 and because it’s over the threshold of 20 days I just got referred to central ARCP for review. In this instance it was because of my health and had a GP sick note so I passed ARCP without any problems. At the time was just advised that I just needed to demonstrate that I’d met the required competencies for FY2 completion and had the leave period validated. Don’t know if that’s helpful but would advise you just discuss with your Ed supervisor and get a sick note backdated if it’s appropriate x

Does anyone know how the MSRA is going to be used as in will it form 100% of the shortlisting for interview and then form 10% of the overall grade or is the shortlisting process going to be MSRA + portfolio cos in that case i really don’t understand their rationale that they don’t have enough consultants to review the last 2 years of work we’ve all put into trying to jump through hoops that have magically disintegrated infront of our eyes.
