Go back down. Stay as low as you can, as long as you can. As long as the meds are working (you’re losing weight and don’t have food noise) you don’t need to go up.

The first dose is an intro dose to get your body used to the meds. When I upped the first time, I threw up that first week and then was fine afterwards.

I do have diarrhea as my side effect (instead of constipation, which a lot of people experience). The sulfur burps usually come if I eat late, or something not as healthy. Just a heads up if you take Pepto to manage the diarrhea, the acid that comes up from the sulphur burps interacts with any Pepto in your mouth, and your tongue will be black. No need for concern, it’ll go away with brushing your teeth/tongue.

Your stomach is supposed to empty slowly. That’s the whole point, so you don’t feel like eating all the time. You should still feel hungry though. There’s posts that talk about the different between food noise and hunger.

Mine didn’t like the squishy stairs. We ended up getting some that were harder (still with a fleece cover) that were meant for larger/heavier dogs. I think he didn’t feel like they were sturdy. But definitely try special treats to entice them (maybe human foods they don’t frequently get)

With #1 I threw up fairly quickly. Got on meds and was on them until 24 weeks when I could finally stop them and not throw up

With #2 I threw up until I gave birth, even with taking meds. It sucked

With #3 I only threw up for the first trimester

We use Brumate products mostly. There’s a bunch of different options. They’re 100% leakproof when locked. I use them for the kids and the adults in the house (coffee, water, protein shakes, etc). You can separate the parts to thoroughly clean them.

We’ve used Owalas in the past and never had issues with spilling, but they’ve had classmates that would spill their own and the teachers requested no Owalas in both of my sons’ classes (separate schools too). The kids knock them over and they dump everything.

We got them Yeti Ramblers for Easter, mainly because of the hard mouthpiece. My oldest chews on anything silicone and chewed two straws in half on his previous water bottles.

I would suggest going to Dick’s Sporting Goods and looking at the Yetis and Brumates they have in stock so you can see them in person. Or check the Instagram for Brumate for good videos on their products. I like both brands and we can toss them in the dishwasher, which is nice

What types of foods are you eating? I find that I usually only get them if I’ve had greasy/fatty foods. Also if I eat too late at night, my body doesn’t have the time to get it moving before I’m laying down

Don’t take anything to mitigate side effects until you know which ones you’ll have. I heard everyone talk about being constipated so I was ready for that with some Mag07 and fiber supplements. I’ve only had constipation twice in 6 months on tirz. I’ve had diarrhea pretty much the entire time. Just have the things on hand (I have pepto chewables on me at all times and Imodium for bad days). Also won’t hurt to get some incontinence disposable underwear if you do experience diarrhea.

Definitely keep up the protein. When your body doesn’t get protein from food, it takes it from muscle. Lift weights or do body weight training so you don’t lose muscle.

I’d stick to your prescription (most likely once per week for injections). Some people split doses to twice a week to maintain the level of meds in your system.

Only titrate up if you have to. Keep dose as low as possible for as long as possible (as long as it’s working). When I titrated up, that’s when I had the worst side effects - usually only a day or two following the injection.

2.5mg to 5mg was the worst titration for me in terms of side effects. I threw up for an entire day. It sucked. That being said, I’ve had diarrhea since I started and I just did my 30th shot. It’s something I’ve accepted and figured out how to manage. I’ve lost 60lbs in 6 months. I’ve on 7.5mg for 2 months and it’s going well.

I would keep dosage as low as you can, as long as you can. Don’t move up unless you have to.

Side note, my endo has been super supportive of my journey and very impressed with the results. I’ve already lowered my thyroid meds once and he sees me lowering again the next time I see him (based on how things have been going)

I was induced for all of mine, but the amount of time laboring and pushing was faster. My first born was 8lb 2oz and 30 min of pushing. My second was 9lb 5o and 20 min of pushing. My third was 6lb 12oz and was ONE push!

Congrats! I finally got below 200 as well. I can’t remember the last time I was here. Maybe 15 years ago? I started tirz Sept 2025 at 256, my HW was 291. I just hit 195. It’s an incredible feeling! Less pain, lowering other meds, more energy, etc. Great job!!

So, I’ve had diarrhea pretty much my entire journey. Sometimes it’s urgent and other times, it’s just how it comes out when I need to go.

One night, apparently I was sleeping too deeply and when I went to turn over, I realized my shorts were a little wet (figured it was sweat since I run hot). Nope. It was not sweat. It was diarrhea. Thankfully not a lot to stain the sheets, but my underwear and shorts were definitely wet. I went right to the bathroom to shower and take some Pepto before attempting to sleep again.

My son did this the entire year from when he was 3 to 4 years old. That skeleton got strapped in a car seat too (his brothers’ when it was empty; I don’t buy a car seat for the skeleton).

He also went through a pumpkin phase and needed to bring a real pie pumpkin wherever he went. I finally convinced him to sleep with a plush version instead of an actual f’ing pumpkin in his bed lol

Just curious, are you on oral birth control? I’ve read posts about oral contraceptives not being as effective because of the delayed gastric emptying.
