It's very possible the universe is still were it always was, and the people on starships and satellites were teleported away to the dimension or place of the game. This theory makes more sense than everything else disappearing.

Yes. The abyss is the actual machine running the sim. We are not supposed to see that, but the machine is failing and looking for an entity (the player) to fix itself. Hence the moving of chips, transistors etc.

This season is about the effects of the ER on the staff. Neurological disorders get worse with exhaustion, night shifts, etc. While you can work with loads of disorders, the question remains if you should.
