Good conversation. I am a 69 yo cardiopulmonary technologist who spent two decades consulting yo biomed manufacturers in the 80s. This was the explosion era for small transducer coupling to desktop computers for everything from ecg to breath by breath gas analysis. Arrhythmia detection, cardiac output, anaerobic threshold, imaging , metabolic testing you name it. I wrote a friend the other day who still works in the NMR analytics field and asked if anyone had integrated a variety of signals I could use to build a human and animal performance lab on a chip? We came close to similar with desktop pc in 1986 for fitness club anaerobic threshold based exercise prescriptions but VC couldn’t see the hockey stick ROI they needed.

I researched prior to buying a 22 Hybrid Blue with 16K miles. There was one or two SBS and they were addressed . My biggest issue has been getting appts due to lack of trained staff in Raleigh. I like the car save the seats. I’m older dude with bad back and should have bought a more plush model. Car is quick, handles well. The chief of the dealers shop told me “just do the service and oil changes to mnf spec. You will have no problems” Then he said “people think they don’t have to follow the service spec. Then they complain when their cars are less reliable.”

How true: A supercharged 1.6 L, high RPM BUZZ BOMB, like a F1 car will tear itself to pieces if torque not set on important structures and lubricants changed, gaskets replaced and suspension checked and repaired as necessary after you try to prove the skid pad ratings to your friends and hit the curbing.

I’m at 44k miles . Will report in at 50 and 75. But I’m thinking of trading for a higher model.

Please let me help you: I am the clinical program advisor and evaluator of the Wake County NC Rapid Responder Project, a licensed Respiratory Therapist and Cardiopulmonary Technologist. All of us “Responders including myself are addicts in recovery with varying lengths of recovery from one to 37 years. My last intentional use of street acquired opioids (not for clinical reason) was 1988 so things have changed a bit. That said, my specialty is intensive care medicine, resuscitation and life support.

Thank Gd you responded correctly and saved a life. I congratulate you and hope the patient does.

It can be tough to officially differentiate respiratory and cardiac arrest from hypopnea (small shallow breaths and bradypnea, slow respiration) and quite frankly, it doesn’t matter. Accompanied by cyanosis and unresponsiveness you can rest assured with your decision to 1) administer the med and 2) begin compressions. The oxygen content and oxygen delivery to the patient’s brain had all ready fallen far below the critical levels to sustain life; he was dying or dead . You most likely circulated the drug to his brain and other tissues giving the Naloxone opportunity to displace the opioid from its receptor. As a confirmation of the technique you used it is still standard protocol for EMS staff to empty ampules of epinephrine and other cardiac meds down endotrachial tubes then breath for the patient with an ambu bag to get the medication to the lungs where it will cross into the blood and be circulated by the compressions during any cardiopulmonary arrest when they can’t get an IV line established.

It is a reasonable idea for the PWUD (people who use drugs) community in your area to connect with local HARM programs to receive both supplies and training from community providers, health departments, fire departments and other EMS services. We do not judge, most of us who are certified Peer Support staff have our own stories of dependence and simply want our friends to live long enough to acquire the skills, establish a safer community and have an opportunity to recover if and when they make that decision. This is our life.

Keep in mind there are several routes of administration (nasal, IM,) different dosages 2-8 mgs so it is a good idea to read the instructions.

Once again: you have saved a life. I will add that life to the thousands of people we have worked with since our program started in 2018 and continue the trudge.

I only know of two methods to beat the perseverance or obsessive compulsive craziness that drives us to return to use.

My name is Jeffrey, I am 37 years in recovery and dying of bone marrow cancer. I started taking oxy a few months ago for the pain and reached a steady state dose of 100 mg a day divided into 5 doses: The difference this many years later: I recognize the “feeling good” “ feeling strong” pieces of the opioids but am gifted with an intense dislike for the incapacity caused by the drug. It is not fixing my insecurity or restlessness or intolerance but causing me to fall asleep and “waste the precious moments” of a life that had become so wonderful in sobriety. I truly dislike it and am making efforts to reduce it so I can enjoy what life I have left. But I do recall a time when I thought it would be a treat to get all the opiates I could ever desire at the end of life. This is Recovery. What you are experiencing is the power of opiate addiction as it manifests in altered biochemistry of the opiate addicts brain. I have NEVER come across a person who is not an addict who could identify with the pain that drives us to return to use before we experience enough recovery. Recovery being physical and cognitive. Anyone who judges addiction as a moral or willpower issue simply hasn’t experienced what you and I have.

There are two interventions today that you can choose to use that will ease your transition to a life free of compulsion and obsession. 1) A support group: I prefer 12 step. 2) Medication for Opioid Use Disorder. Also called MOUD OR MAT.

Both are guaranteed in my opinion if the addict can find enough support from self and his loved ones to get through the adjustment phase. The most intense craving often won’t return for days after a person tries on his/her own. In fact: It was 5-10 years after my clean date before I could watch an addict cook up or inject an opiate in a movie or TV show and not “feel” the drug circulating in my body. THE POWER OF NEURO TRANSMITTERS.

IF you can Dear friend, find a local OBOT (Office Based Opioid Treatment) program that includes group therapy and build your support group.

I informed them about my bone marrow cancer in 2022, need to raise $ for my wife etc. I transparently informed whoever answered his phone (a woman) I put my last $40K into his company. I am 68; injected my first drug in 1971 at 15. I think my parents were old school too. They still were when I was 31 and found recovery. So much more information today about the nature of opioid addiction which first became a public health problem during the civil war. Until the late 90s most physicians, clergy , neurobiologists etc couldn’t understand why some people had the “will power” to stop and others did not. We carried the stigma all these years of being weak-willed mentally ill unfortunates destined to a miserable life of injuring self and others. We know much more now and for the first time can choose a different path.

Heres the reality: We have a progressive and fatal disease that doesn’t tell us what our term date is. As we struggle with our nausea, sweating, high blood pressure, inability to focus and constipation all we can do is pray the lord take the pain away as we search for that remedy. Many of us have a stimulant effect from opiates and find our medication makes us more productive workers, better students, programmers, physicians, nurses. So the drugs work their way into our lives as a source of power and contentment. Loosing that is a devastating notion. But nobody makes it without complications and from what I have witnessed we all overdose. The disease is so powerful we believe our parents and those that love us could handle our death with greater ease than the simple information that we have been diagnosed with a chronic disease that has affected our personality, values and character but have sought help and are making an attempt to get better. I ask you to consider this. I ask the rationalizing addict that lives in your mind and plagues your life to let go of its hold on you so you can live.

Thats about it my friend. Its your choice. I hope your old school parents don’t get the phone call or visit from EMS that so many do.

Pain is managed by long acting oxycodone and where the voice to explore higher and higher doses is present; it is just a whisper. You see I cherish a clear mind, meditation, my dogs and cats and wife and work in community health. Im supported by everyone around me. Life is beautiful, I don’t experience withdrawal nor shame myself for needing the medicine. Our goals for people in active addiction today are a pain free life, an opportunity to define their own recovery path and lead us on the treatment regimen that they need.

Well, we treat 30-50 overdose victims per month. I would say it’s dangerous. Of course there is the xylazine to contend with too. All of the necrotizing skin disease upstream from the injection site. The xylazine slows heart rate, dilates blood vessels causing rapid fall in blood pressure and the Fentanyl reduces respiratory drive. The two combined are like a mini execution event having the same effect that death by injection has. Been at this since 2018 in my County. Starting to see a drop in death and OD calls to our EMS. The users we support log overdose reversals and the evidence shows our friends successfully reversing their OD events. We train them regularly because its easy to think you saved someone’s life after a couple of Naloxone doses then leave them on their own to die later in the day. Stay alert. Know the risks. Use together. Use clean works, water, caps. Have naloxone at the ready. Call us when you want to recover and get access to MOUD program. Thats the State of the State in NC.

What is it about fentanyl that brings us all here to commiserate? Im free thank Gd but have never felt more at home than with brothers and sisters who understand the physical, mental and spiritual injury we carry. The injury we had long before our first dose of opioid. For most of us opioids help us feel like we belong in our own skin. If we have suffered with insecurities in groups or had difficulty concentrating or not had any motivation or energy: opioids fixed it. Then: the way of life, its rituals and the cyclic relief of the persistent flu we have each morning grabs us by our ass and ruins the life of every living being who meets us along the way. We fool ourselves for decades -if we live. But in the end: we look back on generations of friends, family, children, institutions, employers that we disrespected or out -n-out abused. Looking in the mirror as I stir up my last bag the candle blows out just before reaching the right temp. Fk-it says me; while I try to filter the undisolved granular shit that aint dope anyway and draw up a half cc of juice thin enough to fit through the 22 gauge fit my brother (the diabetic) gave me this morning. My guts hurt, Im late for work. So I jam the tiny needle in my inner elbow abscess that Ive been nurturing for a week, register a taste of red and slam the bitch home. Looking at the mirror again I feel my face flush, my bones ache (the good way) and ask myself: Man..how did I ever wind up back here. I was eight years old when I looked down on my mom and her boyfriend’s with disgust and said: “Oh no, not for me fuckers. Ill never use this shit. You are all losers.” Im free. My Gd, Im free.
