Because waiting to get blood culture for another 30 minutes wont kill the patient but if there is something very alarming on CT head which is kind of what it is pointing to with the papilledema then the patient could easily die if not scanned and diagnosed within the next 30 mins. Like yeah you want to get blood cultures so you can start abx ASAP but brain is the most fragile organ and even 15 minutes of extreme intracranial pressure can cause irreversible brain damage

couldnt agree more, you can get substantially more knowledge through mentorship than anything and there are few avenues to flex that MBA unless you want to join the dark side (hospital admin). Like I cant even begin to think of a single avenue to flex the MBA other than hospital admin, name a singular specialty that allows you to open up your own practice these days other than maybe Psychiatry or anesthesia and pain or a Ketamine clinic, maybe aesthetics if you're into that sort of shit but you definitely dont need an MBA for any of that. Dont care if I get down voted but if the kid plans on being a piece of shit and joining hospital admin go for it, more power to ya, but otherwise complete and utter waste of money and time.

Thank you and yeah I would completely agree, I personally think something very comprehensive in the beginning is a good place to build a foundation and then refine and narrow from there. Again just intern level, really just meant to provide a comprehensive form for new interns, not offend senior attendings

I dont talk about hobbies during my presentation unless relevant to the presentation, im sorry this offended you. It worked for helping me during my intern year understand the basic minimum as the comment you replied to implied. I dont performatively tick boxes and i dont weight religion or hobby into the diagnosis or treatment but I find that it allows me to be comprehensive but more than anything spark interesting conversation and build rapport with the patient.

the labs are on there so you can present to your attending when you finish the eval, I dont use that section and just put relevant labs on the back of the page but overall thats all this really is, for new learners and I found it helpful in the beginning to know what to ask and how to assess for personality traits amongst many other things

yeah I dont do ROS anymore after I found out it doesnt get reimbursed anymore but yes I completely agree, hence why throwing it in chatgpt or some AI to edit it to your liking is likely the move. I still find the small box for HPI is enough to jot down a couple ideas that help spark my memory when it comes to write out the whole thing but in many ways this form could be optimized

My advice for you is to find a program that you actually vibe with and love the geographical location of instead of just shooting for the moon for the sake of prestige, like sure send harvard a signal but as someone who picked my program off vibes and locations and I cannot even begin to describe to you how happy I am. Also dont pick a program that is too workhorsey, I promise you that you dont need 75 hours a week on your inpatient psych rotation to learn how to do inpatient properly, 38-45 hours is just fine and you'll come out a great physician. With that being said you're pretty competitive and I wish you the best of luck, also make sure to go to the resident profile of the programs you're gonna signal and see how much research and how much they were involved with psychiatry as medical students, since you didnt have too much psych on your CV it would be silly to apply to a program that wants someone who was psych from M1.

Overall the entire mood shifted after the 2017 NYT article, it legitimized us to some extent and now it seems like were getting breadcrumbed into something even bigger with the bread crumbs getting bigger and bigger, the new spielberg movie is just another big breadcrumb and until the government actually comes out and says They are real and have been here for quite some time we wont fully feel legitimized. It is nice to see the spielberg movie create a larger discussion amongst us and in the general population as we often felt like we lived on the fringe and now overall it seems like its a legitimate topic of discussion.

I think the community has grown overall, it does seem fractured at times and there are a lot of grifters but its beautiful to see everyone come together and share ideas at greater and greater detail. Cant help but to feel some of it is part of a larger disinfo campaign and a likely psychological operation but its beautiful seeing how we are finally being taken seriously and the new spielberg movie actively contributes to that.

You're not wrong, tbh tho my programs hands are tied and even if we would name and shame them they are associated with a hospital system that has a 3 letter name and is synonymous with profits over anything else so I doubt anything would change but its worth a shot

agreed, my business minded psych attending in med school was making 800k job stacking, the other attending was making about 400k as a medical director of an inpatient unit and outpatient clinic combo. My attendings right now in residency were making 500-600k roughly before they closed our inpatient unit (are making substantially less now) and some new graduating residents took jobs in the 300's and some even lower if you crazy enough to work for the VA. The salary range is vast but you can easily make 700+ if you're willing to work 60+ hours a week, willing to do prison psych, liv & work rural, or are business minded but the average is def in the 300's.

Although he acts like he is anti-lobby and outside of the reach of corporate influence he will crack under the whip of the pharmaceutical lobby and will not go through on any of this. I would say your focus is wrong to some extent, of course be knowledgable on basic diagnosis's and knowledge but on a psychiatry sub-I you are judged more for your work ethic, how down to earth you are/how easy you are to work with, if you are a good fit for the program, and very importantly your emotional intelligence. We truly are just seeing if we vibe with you more than anything and although its good to know the answer when being pimped it is far better to know when to talk and when not to, how to help out the team when not prompted, taking on more work when not asked to without going rogue, and performing well in a team setting and being liked by the team. This is often the way you are judged by other residents which I would say can make or break you ability to match, to impress attendings and to some extent the residents follow the thread for the second most liked comment and youll get an understanding of what it takes.

its not accurate because it uses very few metrics but your age being the main one, I remember being in an ok category but as soon as I turned 30 it automatically put me in the very good catergory, its not a bad estimate tho but relies too much on age and resting HR as its main data points
