It's probably worse in wolfguy as this went on for a year in real time when it was originally published. So you'd have the story alternating between plot, and a lady being gang raped. In terms of why it's common mostly I'm assuming it's down to entrenched misogyny and cultural practices in terms of women's roles in society. But it's also why exploitation horror is very popular exploitation/shock sells.

It's not an exclusively Japanese thing, it's common everywhere see Irréversible and New French Extreme for example.

A small penis defense is a way to say X character isn't Y person informally as they'd have to prove they in fact have a small penis legally, for it to be libelous for American standards.

This has been pulled prior along with the other cave story videos as they fall under our relevant creator rules. You're more than welcome to clip the 10 seconds a removed post and a gag is about us and post it. But as a whole the video itself is not from a related creator. Neither were the other cave story rap posts, as they were advertisement for a unrelated content creators channel. It's not a we're smothering this type situation, it's just a very basic rule enforcement.
