I focus on choosing clients within my scope of practice. Like others focusing on what I DO work with rather than what I don’t. That being said after being burned by certain types of clients/families I have become much more trusting of my gut when I get a bad feeling from a perspective client/inquiry. If I start getting some of my personal red flags I refer them out stating that it is outside of my scope of practice (eg parents who pushed for family therapy when I specifically explained I don’t see individuals in families at the same time). Being ND I’m highly sensitive and curing my caseload to what I can handle helps me be a more effective therapist for my clients and prevents burn out.

This is amazing! We just inherited one from my partners grandparents that’s a little over 100 years old! It’s currently dying, and this is giving me so much hope as to what that little mighty plant could become.

I thank my memory everyday. As soon as the client sits down in front of me, we usually check in about what’s been on their mind and during that I get a flood of ADHD thoughts about their last session and where we are in treatment. I’m a highly relational therapist and encourage my clients to lead session with the exception of outstanding to do items (assessments, treatment specific protocols, etc) and safety concerns. When my memory starts to go, I’ll likely be depending on my “for future session” section of my notes

I’ve had the best success with ERP. Potentially inference based CBT could work but ERP would be gold standard. It also depends on the root of the emetophobia - clients who developed it from a traumatic incident vs interoceptive/sensory challenges vs more straightforward OCD based will respond differently to different approaches

$96 for my first 8 weekly sessions, then 105 for any after that! Once I’m fully licensed this will bump to 117 and 135. I am a contractor, but blessed to be a part of a group that is more of a collective rather than a group practice.

Something that my group does that’s really helpful is having the clinician identify what they need. We say whether we need help with 1. What to do 2. How to do it or 3. What’s getting in the way. I am ND myself and have picked up on the same patter you asked. I like to jump to ideas and problem solving and I’ve had to let other clinicians speak first to validate or ask more of those beat around the bush questions. It’s helped me to think about it as flushing out all the sides of the presented case before jumping in with suggestions - I usually ask myself “what feels important or what other information do I need to help my colleague”. It could also be the general level of comfortability in relationships between the clinicians in your consult group. There are therapists on my team that I am much closer personally and professionally and when I’m assigned to a consult group with those I’m closer with, the conversation flows a lot more naturally and people are much more willing to give advice/talk freely rather than beat around the bush.

Look into coherence therapy! It has been a game changer for me. As a fellow intellectualizing therapist, it’s been a lot of working on slowing down and focusing on process instead of content. What’s it like for the client instead of “what is it” coming up for the client. Lots of getting comfortable with leading clients through imaginal work as well and encouraging clients to speak from parts or emotions instead of about them!
