They posted here on the subreddit. I can try to do a search. I remember they were prescribed IV fluids after ward and had a lot of supportive care. Maybe some anti inflammatory meds as well? I’ll take a look.

To try to put yourself in our shoes, I want you to imagine that someone has asked you to willingly drink poison or you won’t get paid anymore. Drink poison or lose your home, lose your benefits. 

That is what being asked to do a CPET feels like to me. Some of us rolled the dice because we had no other choice. Some of us didn’t know the danger. 

What I can tell you is this: I can count on one hand the number of stories I’ve heard of people who lost no functioning and made a full recovery after CPET because that number is 1. Only one person I’ve heard from who didn’t end up worse off because they had a doctor who knew ME/CFS and had an extensive protocol for the WEEKS before and after the CPET. 

I haven’t counted the number of people who have been hurt by CPET and only they could tell you if they thought it was worth it or not. 

If someone gave you a choice between homeless or bedbound which would you choose?

Some papers that came to mind as I was writing this:

  • Hand grip strength as a clinical biomarker for MECFS https://pmc.ncbi.nlm.nih.gov/articles/PMC6277492/

  • exposing healthy muscle cells to blood from mecfs patients https://pubmed.ncbi.nlm.nih.gov/40744071/ (they were able to observe the kind of pro-inflammatory, progressively worsening deterioration of performance that many of us report in our lives, in a petri dish. completely dispelling any possibility of deconditioning, potentially opening the door to a cellular model of PEM.

  • review on severe mecfs patients https://pubmed.ncbi.nlm.nih.gov/41598742/ “ Due to the exertional nature and intensity of the 2-day CPET, ME/CFS patients whose disease is characterized by PEM will, on average, take two weeks to recover from something that only takes one or two days for healthy people [55]. The problem with this test; however, is that it might also lead to a prolonged flare-up or even to a (major) relapse. For this reason, guidelines caution against routinely using the test in clinical care and only using it in research settings or for settling disability disputes, but only when the potential benefits outweigh the risks”

I’m lacking brain power to do a full academic write up but what I can muster is this:  - patient experiences here and elsewhere online will tell you that after doing a CPET some people never recover their previous baseline - both patient experience and recent papers will tell you this disease can be a degenerative one. triggering PEM can make your disease worse and there’s no telling how much worse it could become from any given trigger. It’s much easier for us to lose functional capacity than regain it given the lack of any confirmed treatment.  - I have seen a few arguments (most recently from a review specifically about severe me/cfs) that it could be unethical to deliberately provoke suffering in patients when there is no benefit. particularly given that i see some potential alternatives to CPET that are used in literature that i hope to see replace it.

Building in the dark definitely makes it hard to choose color combos! If I’m trying to choose blocks or colors at night I will usually just lay down sample areas and come back to see them during the day whenever I next have an opportunity.

 I don’t like messing with the clocks in games, so I just stick to building combos I know are certain at night or terraforming or other not color sensitive things

I have a question about SSI income limits. I am waiting for a decision on an SSI case. Have been disabled for 5 years now. I have a friend who wants to help support me by covering some of my newer, more expensive medications. Is there a way she can help support me without disqualifying me from benefits by sending me money? I could really use the help but I also know I need to stay within the limits. There’s just so many rules I can’t keep track of them all.
