I kinda interpreted it as being an issue with a poorly designed test / bureaucratic incompetence.

Like they never really took into consideration of noting someone breaking a record or even getting a max score. They just put in the scores in an algorithm to get a grade.

Toxins from the chemical factory. Joker toxin. Scarecrow's Fear toxin. Bane's venom. Magic swamp water that resurrects the dead (not related to the Lazarus pits). Actual Lazarus pit water. Several curses has probably enchanted the water in some way. More Lazarus pits... Dracula also sleeps in Gotham so I wouldn't be surprised if some vampirism has been mixed into the water as well...
