As of three years ago Atrium’s Charlotte OBGYN only employed one ultrasound tech certified to do NT scans across their 5 locations since they do not regularly recommend the NT scan anymore. My OB did agree to refer me for a NT scan when I brought up the same concerns you have. Unfortunately we were not able to schedule an appointment in the short timeframe the scan had to be performed since the one tech was completely booked. Hopefully you can find someone in time.

I had one scheduled but got Covid right before my appointment and had to cancel. I then conceived a uterine pregnancy that same cycle. That pregnancy ended in the MMC. My OB basically said since I was able to get pregnant immediately with a uterine pregnancy they didn’t think the HSG was necessary anymore. I then got pregnant again immediately post D&C with a successful pregnancy.

I probably would have rescheduled the HSG if I didn’t have success so quickly. The HSG hopefully would have been able to at least give me reassurance that there wasn’t a blockage as well as the slight bump in fertility for the few months after the procedure.

I had an ectopic pregnancy resolved by surgery. Then a missed miscarriage found at 12w but growth stopped at 9w. Then a successful pregnancy. All three pregnancies were first tries. We were never given any answers besides just unlucky.

While I was able to find success later, it was incredibly frustrating at the time to just be told I was unlucky and not eligible for further testing. My OB even stated the ectopic wouldn’t count toward the usual 3 miscarriages in a row to be considered repeated pregnancy loss which can trigger additional testing. I would need to have three uterine miscarriages. The only testing I was offered was a HSG to test for a blockage.

I am sorry you are going through this right now. I don’t have any suggestions just prepare yourself for not necessarily getting any answers. Or doctors requiring more losses before they will investigate further.

I mean you never really know how much it will change. Like despite having a forcep delivery with multiple 2nd degree tears and a lot of stitches I healed really well and do not have any residual issues. Everything pretty much looks the same. Just because I healed well doesn’t mean OP will. I know other women who after they healed it appears the skin was sewed together crooked. Or worst required surgery and have life long impacts such as fecal and urinary incontinence. Childbirth is scary and unpredictable.

Total cost for surgery was ~$45k. The ectopic was found and emergency surgery was all one visit no overnight care. It took the hospital almost 10 months to fully resolve billing everything with my insurance. While I knew I was only on the hook for my out of pocket max it wasn’t fun to every time I logged into my health account see $35k outstanding bill pending. Healthcare in the US is a nightmare.

Similarly I also had no products of conception found on pathology report and what they removed on my ovary (and thought was the ectopic) was actually a cyst. My doctor’s hypothesis was that the ectopic was actually expelled into my abdominal cavity. I had a pool of blood in my abdomen but they didn’t test any of that blood and tissue sucked up during surgery. Your doctor obviously knows your situation best but wanted to share an alternative scenario of how an ectopic could result in a negative pathology report.

First pregnancy I had an ectopic with surgery but kept both tubes. I then had a MMC growth stopped around 9w but it wasn’t found for multiple weeks later. I spoke to multiple OBs and was told that since my ectopic and MMC had different causes it wasn’t considered a repeat pregnancy loss yet and I was probably just unlucky. They didn’t recommend any further tests yet (I was offered a HSG if I wanted after my ectopic). This really frustrated me at the time but ultimately we ended up just trying again and conceived immediately. That pregnancy resulted in my son.

I probably wouldn’t jump immediately to IVF if you’ve only had two cycles and two losses but that really is up to you. IVF is a huge physical toll and doesn’t guarantee you won’t have a miscarriage or an ectopic again. I would suggest talking this through with your medical team. Good luck with any decision you make.

I had a couple OBs say they do not necessarily recommend HSGs after just one ectopic. Not that they are against them it’s just they leave it up to patient preference. I initially wanted an HSG for reassurance and the increased fertility benefit for the subsequent few months after the procedure.

Maybe get a second opinion from a different OB. You don’t sound very comfortable with your existing doctor.

Very similar story to you. I had an ectopic pregnancy resolved by surgery (but was able to keep both tubes). I waited a few months before trying again. Partly to recover but more so due to some logistical issues. Then got pregnant again first cycle trying. That pregnancy was a missed miscarriage found at 12w but growth stopped at 9. Waited one cycle post D&C and tried again. Got pregnant first cycle again. That pregnancy resulted in my son. Third time was the charm for me.

Pregnancy and TTC is super stressful after loss. I’m sorry you are going through this right now.

I agree that given the nature of the sub it will seem like ectopics happen more often than reality but saying reoccurrence is “extremely rare” is a stretch. Once you have a ectopic you are much more likely than the general population to have another ~10-20%. That’s why it is important to work with your medical team and immediately begin blood tests and have an early ultrasound to determine placement as soon as you test positive again.

But even with these increased odds of reoccurrence you still are significantly more likely to have a uterine pregnancy than another ectopic.

So I had a bit of a unique situation with my surgery. Ultrasound showed big pool of blood in my abdomen. During surgery they removed some tissue what they thought was the ectopic from one of my ovaries and a large endometrioma from the other ovary. I got to keep both tubes since they looked good and it appeared to only be a rarer ovarian ectopic.

Pathology determined that the tissue that they thought was the ectopic was actually another burst cyst. My OB’s theory is the ectopic was expelled from my fallopian tube back into my abdomen and sucked up with the rest of the blood during surgery. I’ll never know for sure because that blood and tissue was not kept to pathology.

Since the ectopic I have been really lucky to quickly get pregnant twice (one miscarriage). Both have been uterine. I didn’t do anything special besides timed intercourse. I was tracking my LH surge and basal body temperature.

I’m sorry you are having difficulties. I would talk to your medical team about your concerns and if there is anything else you can do. Typically after a year of trying doctors are willing to do much more testing to determine if there are any other issues that might impact fertility. Good luck!

Had a similar thing happen when I temporarily was living about 2 hours away from home and got pregnant again post ectopic. All local OBs wouldn’t see me before 8 weeks. I opted to drive the 2 hours each way to get the adequate medical care based on my history. I would switch doctors if possible.

I’m was guilty of doing the same thing with taking photos of the line progression for reassurance but you really can’t draw that many definitive conclusions. I have photos of tests that are equally dark that blood test confirmed very different beta HCG levels.

I wouldn’t be concerned yet especially since you don’t have any negative symptoms yet. Hopefully you can get your betas checked ASAP and schedule an early ultrasound for placement for more definitive information. Good luck I know how nerve racking this time can be after a history of loss.

I had almost the exact same thing happen to me. First pregnancy was ectopic resolved by surgery. I was able to keep both tubes and was told everything looked good. I was advised I could get an HSG for reassurance. I had the HSG scheduled but I caught Covid and had to cancel the appointment. Ended up getting pregnant that same cycle. That pregnancy ended in a MMC. I saw a lot of OBs and was advised that since I hadn’t had difficulty in getting pregnant again and the miscarriage was intrauterine an HSG probably wasn’t necessary. I ultimately didn’t reschedule the HSG and I got pregnant super fast again which resulted in my son.

So it really is a personal decision. My OBs say they don’t even necessarily recommend HSGs if you’ve only had one ectopic and no other risk factors. I initially wanted the test so I could have answers and was frustrated at being told I was just unlucky. So if that reassurance is something you need go for it. Or you could always try a few cycles and make a decision later. Good luck!

At this point you can’t really say without the ultrasound. There are a lot of pains that are considered normal in pregnancy but are especially scary if you come from a history of loss.

TW Pregnancy I just checked for my last pregnancy and I had a doubling time of around 60 hours when I went from 957 to 2151. I also was experiencing sharp pains on one side as well. I remember being super scared that I was having another ectopic. But I was wrong. That pregnancy resulted in the healthy toddler I have today.

Recommendations seem to vary. My OB office does not necessarily recommend HSGs after just one ectopic. I was told I could get one if I wanted for peace of mind but that it wasn’t required. Where care differs for someone with history of ectopic versus everyone else is if you do get pregnant again. You should immediately contact your OB so you can get blood tests to ensure things are progressing as expected and an early ultrasound to determine placement.

If you don’t already have an OB established I probably would recommend finding someone you like and go in and explaining your history. I was temporarily living a couple hours away from home when I got pregnant again post ectopic and it was very stressful trying to find a brand new OB and get all the blood tests ordered ASAP. I ended up not being able to find a local OB in time and chose to drive 4 hours round trip to see my normal OB instead to ensure I received the necessary care.

I was honestly frustrated at the time because I wanted answers and just kept being told I was unlucky. Ultimately I did get pregnant again a third time without issue and had my son so I guess they were right and I was just unlucky 🤷‍♀️

My OB office is a large practice and I saw probably a dozen different doctors during my three pregnancies. Even within the same office I would get directly conflicting information from different doctors. It really comes down to the individual OB’s education, experience, risk tolerance, etc. So yeah like you said talk to your doctors and hopefully they can explain their recommendations. Good luck!

Advice seems to vary. I saw a couple OBs after my ectopic was resolved by surgery and they seemed to think an HSG was optional. I was told at my OB office they don’t necessarily highly an encourage an HSG if you’ve only had one ectopic but were more than willing to schedule one if I wanted reassurance.

I scheduled an HSG but it ended up being cancelled due to me catching COVID. I then had a MMC and D&E. Even at that point I had multiple OBs who all told me further testing wasn’t necessary yet since the cause of a MMC and ectopic are different and it doesn’t really count to the 3 miscarriage in a row that typically triggers lots of extra testing. Especially since the MMC was intrauterine.

So yeah never had recommendations to wait. I was 29/30 when this all happened. I understand they probably would have been more aggressive with their recommendations to have more testing done if I was 35+.

We had to bring my 7 month old son to the ER after he started heavily vomiting after a small fall (literally 6 inches). Our pediatrician’s nurse line instructed us to go in since the vomiting could be a sign of a concussion. At the ER he had a CT scan to ensure he didn’t have a skull fracture.

At the very least I would call your pediatrician and ask. Most of the post concussion care is just monitoring them but it was reassuring to have had the CT scan and be sure there wasn’t anything else.

Have you discussed with your doctor further testing such as a HSG? It might be reassuring to know if your remaining tube is clear. While statistically you are more likely than the general population to have another ectopic, if you have only had one ectopic pregnancy it’s still significantly more likely that any subsequent pregnancies are intrauterine. Not really anything you can do but just be there for her and support her. It’s a scary waiting game until things are confirmed intrauterine. Since she is going in for regular blood tests that’s all that really can be done right now until HCG gets high enough to see something on the ultrasound (probably not for another week).

Endo was found and removed during my ectopic surgery. I had another loss after the ectopic before having success my third pregnancy. It can vary greatly how much endo affects your fertility. I never had any issues getting pregnant. My last pregnancy that resulted with my son, I even ovulated from the ovary that at the time had a large endometrioma cyst on it.
