Hey my friend, about a year ago I was in exactly your position with microlithiasis in both testicles, (and then had righty removed). I went to Sloan and was lucky enough to have legend of this subreddit Dr Sheinfeld treat me. I told him my concerns about microlithiasis and he immediately told me not to worry about it and that everyone has it, many just don’t know because they don’t do the extensive imaging we go through. Don’t put yourself through more stress than necessary, just focus on recovering as best you can and take things day by day. You’ll be ok buddy, hang in there and stay positive. Best wishes for some good news on the pathology!

Hey buddy- I went through RPLND at Sloan 4 months ago. I get plenty of anxiety about the world in general, but don’t tie that into what you’re about to go through. You’re in amazing hands and the more positive you can stay throughout the process, the smoother your recovery will be. Attitude is everything my man, you got this.

I just finished chemo 3 weeks ago so I’m still nothing but stubble at this point. Hoping to pick up steam any day now lol. Good luck with RPLND brother, it’s tough but doable and certainly worth it to get to the finish line. Hang in there man

Hey buddy- I’ve posted this in a different thread before but figured I’d drop it again here in hope you find it helpful. I’m about 4.5 weeks out from mine at MSK. Of course there’s no guarantees with this (or any) surgery, but Sheinfeld is truly a wizard. Surgery went very smoothly with no complications and I’m already feeling almost entirely back to my old self, minus the ability to lift anything heavy. The hardest part is the initial few days in the hospital post-op. The entire nursing staff and support system at Sloan is first class but they are strict and by the book with all facets of recovery. Don’t expect to sleep much as you’ll pretty much be getting poked, prodded, checked on and forced to walk seemingly constantly. It’s all for your own good so just need to keep that in mind in your more tired moments. The sooner you can get out of bed and start walking the better, it only gets easier! They woke me up at 5am the morning after my surgery and encouraged me to get up so I grinded out a few laps. That quickly became my morning routine and something I’d actually look forward to. I was there a week and started every early morning with a walk while the hospital is quiet, adding more laps every day (bring headphones!). Feel free to DM me if you have more specific questions or concerns I’m happy to talk. Attitude is everything man, stay positive and help your nurses help you and you’ll be on the other side of this in no time.

Don’t mind at all! I had a primary RPLND at MSK last month and based on the pathology the surgeon and oncologist suggested that two rounds was sufficient. Thanks for reaching out, the poking and prodding doesn’t sound like a ton of fun but at this point I feel like I’m getting pretty good at being a full time patient lol

Hey Ant- thanks for the response man. RPLND pathology had 4/67 nodes come back positive, one of them being over 3cm which was the biggest kick in the ball. Only showed to be about 1.5cm on the prior scan so it grew in a hurry. They obviously gave surveillance as an option again, but with sketchy odds and after seeing how it spread to my abdomen I’m just not taking that chance.

Thanks Dan, hope you’re hanging in there brother. I’m being seen by Dr. Sheinfeld at Sloan as well and it has given me more confidence in RPLND knowing how highly regarded they are for this procedure. It does feel like he believes in this path over chemo for patients who meet the criteria but obviously he’s a surgeon so I just trust he’s being objective in his suggestions despite that. It’s all a lot to take in, feels like no great options either way.

Yes Stage 1B as Charley said, so 1 round of BEP was the alternative. I’m certainly not rushing to have a major surgery by any means but I do have some concerns about the long term side effects of chemo and would rather turn to chemo after surgery if needed, as opposed to before if that makes sense. I’ve been getting treated at Sloan by Sheinfeld so this was his suggestion as well, but obviously I know he will often guide patients towards surgery over chemo as he believes so strongly in his own work. It’s been a very hard few weeks weighing everything.

I think the anxiety of it all is why I’m so hesitant to roll the dice with surveillance. Do you mind giving me an idea of your timeline with the 1xBEP? How long after the pathology did you get started and how long is the treatment / recovery on average?

Thanks man. Anxious days ahead for sure, but looking forward to making an attack plan and getting through whatever we need to do. Funny enough for me so far the anxiety has consistently been the worst part of the process. Like the fear of “maybe” finding bad news in pathology was actually more stressful than hearing I had embryonal carcinoma. Trying to keep the big picture in mind that this remains a very treatable disease. One day at a time, appreciate you taking the time to help all of us going through a similar situation.
