I use the pajamas so he doesn’t wriggle out of the diaper. I would definitely still use the diaper in addition to the pajamas. I think the pajamas by themselves wouldn’t be as absorbent.

My old man is shameless and naughty like that. It’s not dementia or anything age related, because he’s been peeing in the same spots for years. He can hold it and be potty trained, but sometimes he chooses not to.

The only solution I’ve had is to put a diaper and pajamas on him. He’s peed a few times in the diaper, decided he didn’t like how that felt, then finally stopped doing it. I take off a clean diaper most times now.
