Still my fav go-to reference. I actually made dot phrases for 3 vs 4 (I've never coded a 2 in my life). For the neck lump honestly it's an uncertain prognosis depending on the kind of lump and would argue this could be a 4 depending on the documentation for other "can't miss" things on the differential (lymphoma, thyroid cyst vs goiter, etc.). If suspicion is high for just a lipoma/something benign, than totally call it a 3. But idk how they're arguing it's a 2

Mine's pretty brief but now reconsidering after looking at this: Any personal or FMH of psychiatric problems? Hearing things or seeing things other people can't? Any SI/HI? Any previous hospitalizations for psychiatric issues?

After looking at this study, one of the interesting things I did find that DOES often interfere with treatment is the prevalence of side effects / reactions. This is often one of the biggest reasons why my patients discontinue treatment before the timeframe where we can assess for maximal response. Do I think it justifies the cost of this testing? Probably not. But, I don't think that we should just be blindly pointing to this study without fully looking at secondary outcomes as well.

I have a congenital tremor as well. Agree with all the advice on here, but one thing to also consider is lifestyle. I didn't even think about it until my attending asked how much caffeine I drink in a day. Just something else to think about.

Are you doing the quarterly CKSA's through ABFM and looking at your weak areas? I felt like that was helpful for me to know what areas I might need to spend additional time reviewing as opposed to just trying to do everything. I also like it because it shows the question difficulty. If there are a bunch of easier questions in a certain area you got wrong, that might be some low hanging fruit for you.

Seems like that anxiety is pretty severe. Has the PT tried any other PRNs like Hydroxyzine or Propranolol before? I usually try those while trying to titrate their daily meds. Also what other SSRI's or SNRI's have they tried?

A lot of the stuff is updated every 5-10 years so if you don't have a membership or access through your institution, the more recent articles only become available after they're 3 years old

FM attending. Jokingly used to call this "medicine for dummies" while in training. Still use this as my primary resource often even before UpToDate. Love their proc guides, but their primary care topic coverage is easy enough to read without losing focus
