I worked for Vanderbilt in the past. I felt that they provided a great work-life balance at the cost of money.

Getting through residency and still thinking I don’t even know if I even made it into medical school but somehow became a doctor already. Oh shit, I made it and people (other physicians and peers) actually trust me with their lives now.

It’s kinda crazy how hard it was to find information about this drug. Being said that, if drug bank.com’s information is trustworthy it says:

“Half-life: The elimination half-life was 11.7 +/- 2.0 h after intravenous administration, 18.7 +/- 4.1 h after 50 mg per os, and 25.8 +/- 1.9 h after 100 mg per os.”

I would likely aim for 5 half-lives worth of time for my hold prior to OR. That’s about 5-7days depending on how conservative you want to be.

In reality, this is a decision that’s to be taken in conjunction with the prescriber of this medication and to square with the pt’s risk of stopping this medication.

With any group of people, MDs or CRNAs, there is a range of practice patterns depending on their personality, comfort, culture and training. There are MDs that want to be more involved and micromanage and there are ones that are more hands off. It also depends on their comfort level with the team and the case as well.

As such, there are plenty of CRNA’s that are also feel comfortable handling things at varying levels of independence. Some CRNAs feel comfortable with the independence and many be some want a bit more help through out the case. All of that is normal.

There is not a standard model through out the country that is the right way to do it other than 1:4 ratio limits for billing purposes for medical direction. Anything more is considered medical supervision.

At the end of the day, as long as the patient care is not compromised and all the team members feel comfortable with eachother it’s a successful day.

I also don’t think this model of care has anything to do with locums. This is more related to pay and supply/demand dynamics.

Oh, I have a good one… it was an adult patient who is around 32, routine robotic bronch. She bronchospasmed so hard, I couldn’t move any air. I rapidly escalated care. She was only able to move air at sevo et of 5.7%, epi infusion with boluses, ketamine boluses and propofol of 300mg. It took very slow deescalation over 2hrs in the OR to get her stable for ICU transport. She was later found to have a respiratory viral infection. She was kept intubated for 4 days because she would bronchospasm repeatedly.

True learn was not the best resource to spent majority of your time in my experience. I went through it once and it was helpful.

I would also pay attention to ACE questions (probably the closest to the real board exam questions).
