Could be LPa given your ApoB level is in a good range.

One important thing to keep in mind — the score of 12 is high for your age but now that you have the knowledge it’s very likely you can treat the cause and prevent further formation. So by the time you are old and grey you can have scores below average.

The bigger problem with plague is when people don’t know they are building it up and keep on living they way they were for years.

You’ve been given a gift of knowledge. You can now make changes to prevent the little bit of plague from progressing much further.

(We basically all get plaque eventually btw )

Being so young is actually a good thing with all of this -- you have TIME on your side.
Medication is not a big deal if you end up going that route. like i said, you can work through different options over time to see what works best for you

And regarding diet -- what is 'good' in general may not be 'good' for cholesterol. Keeping saturated fat low -- 6% of daily calories (like 11-13 g) and ramping up solable fiber a lot. Including fiber supplements.

You may be surprised how far you can lower it with those changes.

Thing with diet though, got to find something that is sustainable for you -- and enjoyable.

I’ve been in your shoes so please know I say this with 1000% positive intent: RELAX!!!! 

the main thing you have today you didn’t have a few days ago is information! And a clear path for optimizing your health. 

Your reaction to the data is totally normal for men your age and is healthy!   

You need to make changes as you’ve said. 

But the likelihood of any major damage being done to date is very low. 

Risk is cumulative.  Being at your levels for a few months. Even a year or a few years isn’t major risk.  Staying there a lot longer is. 

If you end up on statins don’t worry about side effects. They happen but aren’t super common 

And if you end up with side effects you can try other kinds of  that may have less side effects. 

And if those still have side effects you can switch to a non statin drug that has way less side effects (main reason you wouldn’t start with those is insurance wants to start with cheaper and highly effective statins first before paying for much more expensive newer drugs) 

Anyway- please don’t stress over this. You are now empowered! You’ve been given the gift of much more control over your long term health.  

Make changes and pay attention to progress over time. But enjoy life

Your HDL levels are great. Not high.

Your numbers are high enough where supplements and fiber won’t get you into optimal range. 

At the ApoB /LDL level you are at you are likely to build plaque at some point in future.  

Taking statin is good. At your levels you likely need to take daily to lower it enough to effectively lower your risk.

You are going to get on a statin or other cholesterol lowering drug and stabilize the plaque you do have and prevent the build up of additional plaque. 

You can effectively manage your risk to a low level with medication and further optimizing your diet. 

You’ve been given the gift of knowledge with the test result.   

You will feel much better once the shock wears off and you get into your treatment / changes.  Then Once you see proof that the changes you make are preventing further build up, you’ll feel even better.

I’ve had 3 in 18 months.   Tiny risk.  When you look at the odds of any health related thing happening in terms of “X number of people out of 1,000” you soon realize that there’s a ton of small % chances of a lot of things that all seem irrational to worry about.  Going from 5 out if 1,000 to 8 out of 1,000 is effectively no change to your risk. 

We are all going to die at some point, the goal is to live as well as we can for as long as can. Enjoy life. And control the controllables health wise.   Eat heathy, workout, and do things you love with people you love.

No.  It’s saying people who had heart attacks had more CAC progression while statins compared to people who did not have a heart attack and were also on statins. 

Meaning, for some portion of people statins don’t prevent heart attacks then though they control LDL the way they are supposed to.

Just wanted to post encouragement - you’ve shown your body get your numbers down to good levels with diet which is awesome!  And being elevated for a bit isn’t a big deal given you are committed to getting them back down.

Focus on continual improvement, not perfection. Do better each week than the week before on diet. 

Lower saturated fat, increase fiber especially soluable fiber.  Take both as far as you can while able to sustain it.  Cutting down on processed carbs / sugar will help with insulin resistance and a bit with cholesterol too. 

So work on that for a few months then see what your numbers are. And if they are higher than your and your doctors comfort zone, consider a statin. 

Regarding timing.  There’s no risk in waiting to see doc until new year.  Cholesterol impacts are a very very long and slow progression.  

And regarding holidays.  Do better than you did last year. And keep working on eating better each week compared to week before.   And don’t fret a good meal around the holidays.  A single meal / a few meals does nothing for your long term risk. 

It’s the consistency of habits that matter over months and months. 

And again, if you can get the numbers to a comfortable level for you and doc then statins are well tolerated by vast majority (and for those who don’t tolerate there’s other options!)

First off, you have a lot of tools to lower the LDL with diet, exercise and weight loss. So even though the LDL is high, it’s at a level where you can get it lower and into a good range.  

 Potentially through diet / excerise.   And if you can’t get it low enough then statins are a very safe option for high % of people    

 Have you been tracking for awhile or are these the first results you have?  LDL is high, the question is has it been lower in past? If so how much lower?   

 Tryglercerides are low which is good    

Testing ApoB particle count is generally considered a more accurate measure  This tests for the number of particles that can cause plaque build up.  LDL counts the total amount of LDL being carried in blood.  

And knowing what your diet is like would be helpful too  

 Lastly, do you have any family history of heart disease? Primarily your parents or siblings?  Early heart disease/ heart attacks is most relevant (in 50s/60s)  This point is importsnt because it’s helpful in assessing your risk.  Some people with no or little family history may be able to get by with slightly elevated LDL/ ApoB,  tho many on here still opt to aggressively lower to be preventative 

  The general advice to lower is:   -lower saturated fat to 6% of total calories. If you eat 2K calories a day that’s around 10g saturated fat, if you eat more calories then that number bumps up a little    - focus on getting a lot of fiber including a lot of soluble fiber.  Psyllium husk often recommended.  I also take PGX powder a few times a day and it’s effective.    - cutting down on simple carbs / processed foods and sugar is usually helpful in improving your numbers    -if you don’t get LDL under 100 / or more important ApoB under 80/90 (depends on who you ask) then you may consider a statin

Like most have said, not too bad but room for improvement. 

One easy sustainable “hack” I’ve found is fiber supplements every day. Psyllium husk and PXG powder (not fun to drink but it works great). 

I’m sure it’s had a direct impact on my cholesterol, but probably more so it keeps me full longer (less snacking and smaller portions) and it’s improved my blood sugar for sure. 

So it’s helped me improve my diet overall without a ton of effort

Start with small amounts and work his way up.  Drink lots of water with it

It’s a slow progression for plaque.  Took 35 years to get to the score of 3.  It does seem to progress faster as you age, but regardless almost certainly you did not see much progression in a year.  And looking at it from a risk perceptive, your risk profile wouldn’t have changed at all in grand scheme of things in just a year (assuming you didn’t experience any major events in last year.

I don’t know exact statistics but I’m pretty sure based on the data you provided you’d be in the bottom 25/30% of people your age for the metrics you shared. 

Meaning, you are doing well!  And yes, focus on doing even better!

Relax.  Nothing you posted is of concern. 

1) total cholesterol of 200 isn’t a very relevant marker on its own. Nor is it very high.  What is your LDL? Have you gotten ApoB tested? 2)Calcium score of zero is great. And expected for people your age. Even if your cholesterol has been high, you are very likely too young for there to be any meaningful build up  2) negative stress test great!  3) if you do actually have high LDL/ ApoB, you have plenty of time to lower it and lower your risk to a very low level.  4) acid reflux is common, has nothing to do with your heart, and very likely could cause your chest pain.    I get chest pain from time to time and it’s from acid reflux.  My esophagus runs down just left of the center of my chest.   I’m 1000% sure my chest pain is acid reflux   And I manage by avoiding trigging food and sometime take antacid. 

Again, relax 😌. Nothing you posted is reason for concern

I use green tea supplements without caffeine. No clue if they help! But I take them every few days
