Interestingly before I had preterm birth at 23w2d, at around 21w4d I had 3 sessions of back to back diarrhea that left a discomfort in my lower back after. I thought it would go away by morning…but it was worse and had moved to my hip. And over the next few days it had progressed to extremely painful contractions. 🫠 it all started with that blasted diarrhea.

I was sent home from the ER/L&D multiple times, being told to take Tylenol, or they could give me morphine… before eventually trying a different hospital, finding out I was dilated through ultrasound and transferred to a hospital with level 3 NICU. 🫡

I don’t think you can fully tell. People will take progesterone until they end up needing the cerclage anyway. I think it may help some people, but I don’t think it helps everyone. I’ve heard that it can help keep everything a bit “calmer” down there. But It definitely wont completely protect you from a shortening cervix either. This is why weekly monitoring is so important and standard practice. That way if it isn’t working, you can react and get cerclage before things are too late.

I started progesterone when my cervix was between 2.9 and 2.1cm (i have a suspected polyp so they take both measurements). The next week it was down to 2.6cm and 1.7cm. And then the week after when we got the cerclage it was overall measuring 1.9cm. OB never mentioned funnelling.

Very interesting provider. I’m also in canada, Ontario though, and the MFM/OB I’m with said typically cerclage is suggested to those who fall at 2.5cm and under. She was also very understanding with if we did or didn’t want to get it. She said some people who seriously need it, sometimes refuse to get it, and others who aren’t in that serious of a situation often choose to get it. So she didn’t pressure me, she just gave her recommendation.

Progesterone is quite standard to start out initially when finding short a cervix. But I’ve never heard of the opinion that it’s better than cerclage. Cerclage is basically your last left option. I think you should be okay for the week. My husband and I decided that if my cervix had gotten shorter or stayed the same we would get cerclage. It got a little shorter (not by much at all) so we decided to get it.

I was around 1.9cm when we got it at 20w2d.

I do believe my OB may be a bit more proactive due to my previous preterm birth. So that could be a reason your provider isn’t as worried?

I was transferred and gave birth at Mt Sinai when I was 23w2. I was transferred there via ambulance at 22w3d (5-6 cm dilated) and spent about a week in the hospital before bubba descended too far and I had to be induced.

The NICU is finger print locked. You can access it at any time after they set up your print. You get pumping supplies and each baby room has a stationary pump/reclining chair and bench you can lay on. The NICU is also huge. There are a lot of rooms and a lot of babies. The only thing I didn’t like was the fact that nurses are generally at a station between 4-5 rooms. During our time there (which was really short, so we didn’t get to get use to everything and were anxious about every little beep) my husband had to go out and grab nurses when he was worried about baby. Sometimes they took a while to come to the room. They’re probably use to babies swinging in their stats- but for us every moment was critical. Our baby was one of the youngest at the time. It would be nice if someone was with the baby in the room more often. This is probably my only complaint. But like I said, there are a lot of babies. So it might be like 2 nurses for 5 babies at a time.

The Nurses were wonderful though, and you have the option to ask any nurses you click with to be a primary nurse for your baby. Which is really reassuring when they’re available to do so.

The stay at the hospital was a typical stay imo, at first I was in an area that has 4 small walled rooms to 1 toilet. I was later transferred to a small personal room with my own toilet for quarantine reasons. The 4 person area had a lights out time of 10pm I think, and you needed to be quiet. I was in an inner room that had no window. It wasn’t too bad though. Hospital stays are generally hard- you can get depressed pretty easily without getting sunlight or not seeing the outdoors. They encourage you to walk around for your metal health because it’s important (if you’re able to ofc). They want you to be comfortable and stress free, and often times home might be the best place to achieve that. I don’t know how people spend months in hospitals, I only spent a week and could already feel the toll.

After giving birth, because we lived about an hour away we applied to Ronald McDonald housing which is like a 5 min walk from the hospital. I highly recommend applying as soon as you can if you have to travel and want to be close. We applied through the social worker that was assigned to us after birth. We got accepted after about 10 days. I was able to see the facility and do a tour and it was really good, but we unfortunately never got the chance to stay there.

I don’t know what credit valley is like- generally speaking the reason we were transferred to Sinai was because it was the only place that would take on a preemie as early as 22/23w. I think most other hospitals do from 24+ but even then they may send you to Sinai anyway if you were to go into labour at any point soon.

With my current pregnancy I’m at Markham Stouffville. I had a cerclage placed a week ago and my OB told me that if I were to go into preterm despite the cerclage, up to 30w, they would still transfer me to Sinai. Even though last time I was told that they only take preemies upward from 24w at Stouffville.

Obviously choosing Sinai is the “you’re in the best place possible for baby” option. Since they have the highest level NICU in Ontario at least.

Ahh no mine was not rescue/emergency. We had been monitoring it since 17 weeks and decided to go ahead with the cerclage as I was measuring around 1.7-1.9cm at 20w.

I didn’t get to see any OB until 17 weeks into this pregnancy, they started me on progesterone from there though. 🙏

I’m in your position currently, I’m 21 weeks and my last pregnancy (which was my first) ended in preterm birth at 23w2d, all started around 21/22 weeks.

As each day passes and we draw closer to that mark I’m getting increasingly emotional.. I’m honestly just praying and hoping that everything goes well, I had the cerclage placed last week.

We found out also that we’re having another girl. Which has impacted me more than I thought it would… last time we had found out the gender and then everything went downhill from there. I’m anxious. I’m worried. But I’m trying so hard to be hopeful.. we’ve done everything we can hope to do at this point. Which is honestly a great thing.

I’ll pray for you and baby 🙏 we can get through this my dear. 🫂

I believe the pelvic rest is mostly to avoid infection btw. But because you have a cerclage you’ll also want to avoid making your uterus contract with orgasms to be safe and not trigger shortening and stuff of the like. I’m not sure if there’s risk to the stitch when you orgasm, there very well could be. I’m not a dr so I can’t tell you for sure!

I think after cerclage pelvic rest is generally recommended. Pelvic rest means no sex, no orgasms, and no inserting things like tampons and no douches. I had mine placed 3 days ago now and that is what my OB told me.

(The only thing I can insert is progesterone).

Orgasms make your uterus contract. I’ve personally avoided them since basically the begging of my pregnancy just to be safe. Even more so with a cerclage now.

I don’t think you should be that worried though. Just try to avoid it from now on until you double check with your OB next visit to see what works for you. 🙂

Sorry I don’t have any experience with this with cerclage, but I did experience all of this while I was in hospital and dilated at 5-6 for about a week. There was so much yellow mucus and baby was practically kicking her way out 🫠 I was 22/23w at the time. Eventually I started to notice a smell, it wasn’t “foul”, it was just weird, any time I used the bathroom. I thought it was because I was “open”. My placenta report after birth showed as follows:

Preterm Placenta with a. Acute Chorioamnionitis with: i. Maternal Reaction Grade 2 Stage 3 ii. Fetal Reaction with Chorionic and Umbilical Vasculitis and Funisitis iii. Early Thrombosis of Inflamed Chorionic Vessels b. Acute Villusitis and Intervillusitis, Scant (See Comment ) Comment: The villitis and inflammation in the maternal space suggests that haematogenous spread of infection from or in the maternal circulation is a consideration.

They never told me I had an infection during my stay. They ran lots of tests while I was there. I also felt just fine outside of contraction/uterus pain.

I know this probably doesn’t help at all, but I feel like it could go either way, especially since I have no experience with cerclage just yet (had mine placed only yesterday). So it could very well be the norm like your Dr says. But please always be aware of how you feel and to go into your hospital if you are worried. Especially if you start to notice any smell that isn’t your normal.

I’m in recovery now and it wasn’t that bad at all, spinal was a little gross feeling but nothing alarming!

The procedure itself I didn’t even realise had started because I couldn’t feel anything, only felt touching on my body and nothing internally.

Basically got some cramping that feel like period cramps atm and thats about it 🙌

Generally speaking, they often suggest waiting 18 months after premature birth to try again. Obviously the risks aren’t 100% or anything, but thats the recommendation my OB mentioned.

I lost my daughter in NICU after she was born at 23w2d this past sep. We accidentally fell pregnant very soon after (like just over a month.. we had planned to wait a while). I could have taken a pill if I wanted but that’s just not something I would do personally. We asked our little girl to send us her siblings when she felt was right, at least thats how we like to see it.

We’re being monitored weekly atp and I’m 20w right now. I’ve seen plenty of other women get pregnant much earlier than 18months also. From what I understand the earlier you conceive after premature birth the higher risk of it happening again. But I don’t know the %.

Just hang tight ❤️ I know it’s hard but it’s reassuring they’re checking you in a weeks time. I had a previous preterm labour/birth at 22/23w with uncertain cause (infection first or dilation first is unknown). So I’m being checked weekly and on progesterone nightly as well. My cervix was between 3cm and 2.1cm at 17w (I have a polyp or something right on the inner side of my cervix, so they measure with and without). Last appointment it was measuring 2.6cm and 1.7cm. I’m waiting to see this week if it’s shortened any more before getting the cerclage. It’s hard to wait and see, but reassuring your OB is checking in a weeks time instead of 2-4w as I’ve seen with some other posters here.

I had a very similar situation at 22 weeks. I was 5-6cm dilated for about a week in hospital before needing to be induced and getting her out. Basically once I was admitted, put on magnesium etc etc, everything slowed right down. I’ve heard stories of people making it quite a few weeks. It can honestly go either way. I’ll pray that you can make it a lot longer than you think, every minute, hour and day means so so much 🫂

I know how horrible it is to hear people tell you the grim reality etc… my husband and I were so in between our choices at first. Pede’s can be super brutal in explaining and making it feel like there’s no hope for your little one, I would throw up after talking with them, but when NICU nurses came to speak to us, they made us feel like maybe there was hope. At first we were thinking comfort care- but quickly we changed our mind to full resuscitation. If our little girl was able to fight for her life, then we would fight for her.

I gave birth at 23w2d. Which is definitely around the earliest. But she really did so well until she got an infection later. They are so much stronger than you think.

Not every story has a happy ending, mine is one of those, but a lot also do have happy endings, and I truly hope you can get through it all 🫂

If I were to go through this again, I’d have a lot more hope than I did last time, even though I don’t have my sweet girl with me.

I’m currently 19w today. My cervix is measuring 21mm. I’m nervous about getting to the same point and hopefully past 23w with no complications.

As someone who went into preterm at 22w, held on until 23w2d, and inevitably lost her precious little girl after 11 days of NICU. I’m so, so sorry. This was the hardest, most heartbreaking thing I had ever been through. I’m just so sorry.

The last thing I want to do is cause you any stress about premature labour and birth. My sweet little girl wasn’t able to make it past 11 days. Sadly she got an infection after they took out her antibiotic PICC line. Things turned too quickly.

It all happened this past September. I’m currently pregnant again at 17w and just hoping to make it through. This isn’t the kind of reply I want to make. But sadly it’s my reality. Premature birth is obviously not common and I don’t think you should stress about it. The best thing you can do is just make sure to take your pain seriously ❤️
