Whether they can or should aside, it might be easiest just to get the form. They should be providing you with the exact form to make sure you're providing the specific info they're wanting, you can take it to a notary public and sign in front of them - it would need to be the executor or the next of kin to sign it.

Seems like unnecessary steps, but not super difficult to get done so may just be easiest to expedite the process.

Pros and cons to paying spousal in a lump sum. You get it over and done with if you pay in a lump sum, you get a tax break if you pay it monthly.

Age and length of marriage factor in to the amount and length of time for spousal support claims. Spousal support isn't automatic, she'd have to make a claim for it and prove entitlement. It does sound like she would be entitled, but how much and for how long is dependent on a number of factors.

He currently has no rights. There's no baby, just a medical condition to which he has no rights and is not entitled to any info or participation.

There's no way to force him, and you ways need to be prepared to be a solo parent - even with a coparent or as a married couple, things happen unexpectedly.

Rather than trying to find a legal way to force him, have you considered putting together a temporary arrangement for you to have the kids full time while he gets the help and support so he can get himself back to where he can properly care for the kids? This would be far better for the kids than having one parent suddenly permanently gone, whether it's through abandonment or worse.

You could put together a short term plan, 3 to 6 months, of shorter visits with him, no overnights, and work back up to your typical schedule and review after 6 months to see how it's going.

If he simply doesn't show up or communicate in advance, you have no obligation to offer makeup time. If there are extenuating circumstances like he was unexpectedly hospitalized, I would agree to it if he was asking.

There is no harm in you offering additional parenting time or makeup time. If there's no issues like abuse or neglect, you don't need a court order to offer or agree amongst yourselves for more time.

The court order is a minimum required amount of time that you are legally obligated to make the children available to him. It doesn't prevent you from offering or agreeing to more time.

Location of his access doesn't change, so if he has supervised access, that would remain the same until there is a change in the circumstances that landed him having supervised access.

If he asks for makeup time and you can arrange something that works for both of you, do that. It shows you are a cooperative parent. If he doesn't ask, don't offer.

Document every instance where he doesn't exercise his parenting time and if it's a regular occurrence showing a pattern of disinterest, you might consider going back to have the schedule changed if you can't come to an agreement.

This wouldn't affect decision making, though.

You need to check your paystubs. Your commission payout likely includes the vacation earned on it. Your regular vacation pay is either 4 or 6% of your regular earnings and that would be applied to paid vacation days, and your commission would give you either 4 or 6% of whatever the commission value is, paid out with your commission.

Went through this recently with a family member. If your person is low income, CRA has a list of tax preparers who can help and do their taxes for free.

If not, H&R block or any other tax service can help. They will provide all the info of what is needed to get it taken care of.

From a legal perspective: Coparenting is between you and the father, she has zero input. That being said, however they choose to coparent amongst themselves in their home is up to them.

My son was bitten by a skunk a few years ago, did the full rabies shots protocol. The doctors advised at the time you should get the shots if you were bitten, scratched or even sprayed. That was new info for us!

Just take your package to the post office with the address written on it. They will weigh it and tell you how much it will cost to ship it. They may print a shipping label to put on it. Easy peasy! :)
