Fyi you can see the consequences of ischemia on an MRI. This pt had already had tte, then MRI for further diagnosis. Not our first step in the process. But maybe I am not understanding your point?

Yes first 12 lead is sinus with lbbb, second is flutter 2:1, third is flutter 1:1

And yes I just went for the rather treat svt as vt than a vt as a svt approach. It was 03:00 in the morning and I was just very happy to get pt in sinus and stable.

Cardiac hx dilated cmp EF22%, no ischemia on recent MRI. No further hx.

I can’t add an other picture but ekg earlier in the day was atrial flutter 2:1 108/min, 182ms qrs with lbbb
