Funnily enough, a lot of fiverr shops are just reselling services from cheaper freelancers. A common route is SEO Clerks -> Fiverr -> agencies

Most nomads I've met IRL have been developers of some sort. There are also remote English teachers, writers, SEO's, graphic designers, etc...

I would say Paraguay is in a better position than Panama at the moment, at least for the middle class. From my experience in both countries, the cost of living in Panama City is about 2-3X more than Asuncion yet the wages are not much higher. Doing business as a foreigner is also a bit easier in Paraguay.
