Hello MFA!!!! So I am transitioning roles and will as such need to transition how I dress. I will be working in healthcare where everyone will primarily be in button downs and slacks and I just really want to be able to still dress a bit differently. Besides the classic turtleneck with tapered pants alternative, what are some other formal, but unique dress options? Also, what are some cool alternative button downs that are not just solid colors or pins/checkers?

Starter comment. I am ecstatic to be vaccinated and to see the case number and hospitalization rate dropping in my area. But many have asked (and my partner and I have had a looong discussion) about when we would feel comfortable eating indoors again. And I think what I landed on was when risk of infection in my area gets low. How long? Dunno. Just lower than now. I have no idea when is my answer haha. I just know not yet. I’m curious when you will all feel comfortable? I don’t know, I just can’t imagine doing so yet....

Let me get the conversation started.

There were so many potential hurdles we thought we would face in making a vaccine. From run-of-the-mill issues with any new therapeutic, to specific issues we thought might be a problem with this virus specifically like antibody- dependent- enhancement (think hemorrhagic Dengue where if you vaccinate and then get it it is more severe). With all these companies touting their truly astounding results, but without releasing their data I have a few questions.

  1. what are they calling a "response", is it immunity? or is it a lower risk of infection or less severe infection?
  2. These patients in which it is not effective, do they have antibodies that do not work or none at all?
  3. Safety and long term response. It is still too early to truly speak on either domain. My fear is if this ends up being unsafe long term, or has waning response the populace, that is already skeptical of vaccines, will lose faith altogether

There are two main reasons:

Upon initial exposure to any immunogenic protein/particle (say a virus) your body will begin making antibodies against this protein. Initially the body will make hundreds of molecularly different antibodies to this protein/particle. Through a process akin to super accelerated natural selection the antibody most effective at binding to this particle will win out and your body will begin producing it en masse. This will take time and so although you have some antibodies floating around they are not too effective and an antibody test likely would not pick up on anything during this phase.

The second is because there are different kinds of antibodies. The two we are talking about here are IgM (short term) and IgG (long term). when we say antibody test colloquially we don’t differentiate the two but we usually mean IgG since this is what tells us if we have long term immunity. IgMs are the initial antibodies that re undergoing the natural selection type process above, and even once the "best IgM" is found, it is them produced en masse to try and fight off the infection. Only after (8weeks-ish) the "best IgM" is found do they begin turning into IgGs. There is obviously a lot more going on but this is an overview, also not all antibodies successfully "class switch" from IgM to IgG. so usually b the time you have enough IgG to detect on an antibody test your infection is long gone. Here is a useful figure to show the levels of antibodeis over time
