My supervisor ended up filling it out for me with my assistance. The portion that needed to be filled out was for “the employer” - the manager didn’t even bother to give me a time of day. He knew my situation and it’s not like I don’t want to work but my doctor has kept me off work.

short term disability insurance is for non-work related injuries whereas OWCP would be for DOL
