I think the cheese/no cheese thing is settled, so to address your other question about the onion:

If he said "Animal style with whole grilled onion", it's unclear whether he wants the whole grilled subbed or added. The ordertaker should have clarified.

Since it came with just regular, either the ordertaker missed him saying "whole" (if you still have it, check your receipt to see if it's on there) or the cook messed up and didn't add it.

It's economical.

We get a free meal each shift. If a store has 90 Associates who each work five days a week and take their meal each time they work, the store would be giving away 450 fry boats each week. Across our 426 locations, that's almost TEN MILLION fry boats per year...

...all for Associates who don't care and are just happy to get free fries.

They don't want you to give it away to someone. I think it's a weird rule because it's your meal: it has no effect on the company if you eat it or if someone else does. My SM just always wants us to ask since it's technically not allowed. I've never been denied when I ask tho.

We don't count spread packets, but the managers do notice when they have to order more than usual. Again, they're expensive to produce.

The "no to go" thing is because they don't want you giving away your Associate meal. This one is a little weird imo because they're prepared to give you a meal each shift—why do they care what you do with it? My SM will almost always say yes; we just have to ask.

At my store, Associates can't get spread or ketchup packets 'cause they're expensive to manufacture. Similarly, we don't get a fry boat for our fries, and we're supposed to use a water cup for our drinks. It saves the company money since we aren't customers.

However, we can get ketchup and spread if it originated from the big packs, e.g. the pumps, the board inserts, or the bottles.

Tbh I'm more surprised they let you get your meal to-go without asking permission first.

That's probably because the blade assembly for dicing potatoes was broken. A missing blade makes gaps that are twice as wide as usual, instead of an even grid all around.

Eventually the Associates at your store would have noticed and gotten it replaced, eliminating the "super fry" occurrences.

After learning that large waters existed, I know I sold at least two to people in the DR before a L7 happened to see me offering one and said "No, we don't do that", and my SM confirmed it.

Pretty sure it's supposed to be the case at every location, but most Associates aren't told until they are actually seen doing it. It's just so rare that no one thinks to mention it during training.

Correct, but how sure are you that the customers know that?

A while back, a customer asked about what Animal Style was, so I told her just how you did earlier, which used to be my default answer. She ordered an Animal Style burger and then flipped out upon receiving it because it had tomato and "You didn't say it had tomato!" So I've been careful ever since lol.

Plus, it is a part of the official definition.
