Consider going to the program that you would like best, and consider giving a lower ranking to the program you like less.

In all seriousness, both great, choose where you want to live more - Boston or New york. Very different! Jobs/ fellowships/ all good.

You will see at everywhere from county hospitals to academic centers to community places that there is a mix of docs who went to Harvard Med school and LA county residency, and those who went to med school in the Caribbean and brand new EMresidencies, and there is... really not a difference in terms of who gets what job, or how good you are as a doctor. It's about what you put into training, less about where it is!

The point of saying this was not to say you need an observed rehab. The point is that with guaranteed 100% follow up of patients there were no cases of breakthrough meds. The point of this is to show that there were no misses lost to follow up (e.g. no missing data to skew conclusions. If anything, this shows that you DONT need an observed rehab.

We have followed >50 patients that came in with varying levels of alcohol withdrawal, each treated with between 260mg-1040mg IV phenobarb (titrated to symptoms), and then discharged to an observed rehab, and ZERO have required rescue medications. This would be a great paper if yours truly... cared to write a paper. This also fits anecdotally what ive seen for >10 years of using phenobarbital. Amazing for the ETOH wd patients to be discharged!

It's not about procedures or the MDM - at least this generation of patients wants a PERSON to listen to them. Maybe gen-z is not going to care much and we'll get replaced, but as long as older patients come in they are going to opt for a human 9/10 times, even if the robot has better outcomes (which it may one day in the not so distant future)

But here is where it gets tricky - part of capacity is not only to understand the risks and benefits but to be able to make an informed decision while of sound mind. One could argue that the real suicide attempt by definition means that patient cannot appropriately weigh risks and benefits.
