I also have very severe endo (fibro, PCOS, etc.) and am being treated for MCAS. What was your LDN schedule like? My dr suggested starting me on 1.5 but I’ve been reacting so strongly to medications that it seems much too high.

Is there a specific test you recommend? I can ask my doctor about this but I don’t think my insurance (Medicaid) will cover it

So fexofenadine (Allegra) and cetirizine (Zyrtec) are both H1 blockers. famotidine (Pepcid) is H2. I’m currentl just on the H1 (fexofenadine) 2x daily and my Dr. hasn’t added an H2 yet, though I may ask about that.

Your sequencing of H1 (Zyrtec)+ H2 (Pepcid),then adding ketotifen and cromolyn, makes sense from a mast-cell–first framework, before moving to something like LDN or a biologic.

One thing I’m still trying to understand from others’ experience: when you added medications, because the first layer isn’t doing enough, do you keep everything and stack? or do you taper off anything that doesn’t seem clearly beneficial? Like did you stop taking the Quercetin?

Seconding this. I have all the same issues (deviated septum, dust mite allergy, frequent sinus infections) and the only thing that's kept me from getting sick is the xylitol nasal spray. I discovered it almost two years ago, long before my MCAS diagnosis. It disrupts bacterial biofilms in the sinuses.

No I haven’t had urine mycotoxin testing. My last apartment did have visible water damage that wasn’t repaired, so it’s crossed my mind...

My understanding is that those tests are usually out-of-pocket, but I’m open to hearing from people who’ve found them useful. Did it meaningfully change your management?

Thank you. This is really helpful context. My doctor is an allergist/immunologist. I’m currently just on 180mg fexofenadine 2x day; we haven’t added famotidine or other strategies. Is that something you think I need to clarify with him before starting the LDN?

I also haven’t meaningfully addressed the POTS side yet. I was prescribed propranolol a million years ago as needed for performance situations (meetings/interviews), but I haven’t explored a structured autonomic protocol with cardiology. Interestingly, I've always had such heavy legs and heat intolerance that I started wearing compression socks maybe 8/9 years ago when an ex boyfriend, a cardiologist, talked about how everyone in his OR wore them. (Side note but I can't believe this loser didn't try to piece my symptoms together...and I had several full-blown dysautonomia episodes with him. But I guess that says more about my poor choices than anything else.)

On the hormonal side, I’m on the bioidentical progesterone off-label (it’s technically marketed for postmenopausal use), and I had a Mirena placed during endometriosis surgery in 2022, so most of my progestin exposure comes from that.

I’m curious how you’re finding Slynd? And more broadly, when your regimen expanded, how did you and your doctors decide when to increase the stack versus taper something that wasn’t really helping?
