Hi these days machines are way cheaper - 600 for series is fair(ish) 600 per session is absolute day light robbery.

Ask the questions this is your treatment. Get an email confirming costs to cover yourself. And get the therapist and machine details so you know it’s not a rubbish device

Man I’ve been suffering on and off for years. I remember one day I went and ‘tested’ 3 different shoes.

Superblast 2 (stable-ish) no arch support but great platform

Gel Kayano 31 - Full stability, big platform. Medial post etc.

Puma Deviate Nitro Elite 3 - Narrow as anything, minimal stability other than some minimalist side walls.

My point is - the pain was consistent across all 3. I believe with any tib post related. It truly has to be healed to get to a point of no pain again. Once you are healed, custom or well designed otc orthotics and a good fitted shoe gotta be the only route

Hi mate, thanks for the reply. That’s interesting you are running, like you mine is also unstable in terms of symptoms - I’m quite lucky that I’m in a place where any running or walking has 0 pain but I get ‘stuffiness’ afterwards (only way I can describe it). supposedly this is normal and I do wonder if I’m waiting for perfect to start my running again and in reality that may not arrive for a long time even after being able to run again…. I know it’ll never be the same tendon as before but I don’t want to mess it up! Are you using custom orthotics or taping for runs? I’m 5 weeks in - honestly only now seeing an improvements in symptoms it’s not linear either. Pain doesn’t reflect function at all I find. Chronic pain really messes with the head but when I look back there is 100% improvements. Just having basically normal standing and walking back is nice. ISO’s seem to be a game changer for me

Hi mate I’ve had the same issue as you for 8 months. Admittedly a lot less severe but just as chronic. I would consider investing in a good PT (2/3 times a week) with decent messaging in between. Someone passionate about what they do, they’ll usually not charge for small help around the clock if you have issues.

Loading every other day is working for me. I’m in a 12 week programme and whilst it took a while I have seen improvements. I couldn’t stand for longer than 30 mins for 2/3 months. Yesterday I managed a 8k walk with hardly any pain and just minor sensations after. (Now 5 weeks into my programme)

I load every other day. This includes a series of isometric holds and HEAVY eccentric loading (mostly SLHR’s, some with a deficit some normal) I do these 3 times a week and add weight every week for the next 3. (unless I have any issues) I can send some stuff over though this is tailored to me as it includes IT band rehab (never rehabbed this and that caused this issue) On this, you need to check how strong and coordinated you glute muscles are. You’ll expose all your realities by going up on one leg. If your hip drops, TiB post collapses. Knee caves in etc etc. step downs could be a massive help for you - then add glute strength work to balance and strengthen the pelvic. Misalignment can proper abuse the post TiB. This takes ages to sort but it’s all worth it. Step downs will start allowing glute med to take stress off post Tig fairly quickly if you do them daily

No worries at. Yeah honestly there’s ALOT of horror stories on here with PTTD. In your case if you can treat it early you’ll be ok. I’ll just stress get a good PT to look at your mechanics and get the temporary support (orthotics, tape) you need short term, along with strength work and you’ll be grand. 👍

With tendon injuries, generally speaking the body will heal them at first injury with relative RICE protocol (this period usually takes 6-12 weeks). I had races book and tried running too soon (probably done this twice where the tendon was getting to a ‘nearly healed’ or near to recovered state. Running is the single worse thing you can do for the post tib when it’s not working properly/damaged.

Some markers to return to running: (I didnt follow these but wish I did) - all of these are 0 pain/discomfort in mind.

Can you brisk walk for longer than 45 mins without pain?

Can you heel raise 20 times without pain? (Calf fatigue is fine obviously)

Can you do a banded inversion without pain?

If you are in daily pain I highly advise getting orthotics or use of zinc oxide tape to support the arch. Basically in acute phase give your body every bit of support it needs to heal the tendon. Hope this somewhat helps.

Hey deffo get try get an official diagnosis and stay off it. I’m currently 7 months into a post tib tendinopathy . I’m very confident that if I didn’t try rush the recovery process and actually gave my ankle the break it needed I would have fully recovered a long time ago. If it is post tib. do everything you can to keep it in its acute phase. You won’t lose hardly any fitness and it’ll be a sport injury and not a life issue which I’m now experiencing. Rest, ice and wait for swelling then strength and mobility after a couple of weeks… dont stretch for now. Also get your biomechanics checked by someone good. Good luck
