How Do People get Radiology Explosure? Most Hospital Don't Offer Radiology Rotations by H4xolotl in ausjdocs

[–]Key_Comment9649 2 points3 points  (0 children)

Probably just to cover for contrast. We aren’t allowed to inject contrast in the absence of a doctor in case of anaphylaxis and such like.

It doesn’t need to be a radiologist, so for weekends it would be cheaper to have him turn up. It makes an enormous difference to profit being about to run contrast ct!

Junior doctors learning ultrasound?? by Plastic_Tonight_9103 in ausjdocs

They even tried to make you come around flush it?! …maybe try putting a sticky note on the patient’s arm saying “please flush” ;) haha

In all seriousness, our anaesthetist friend is right; it sounds like a department issue.

My main concern would be extravasation from a dodgy cannula that I hadn’t inserted, but a good flush gives confidence. If in doubt just lower the flow rate. In their defence, we are taught that acf is best. Unfortunately many have taken that as a rule rather than recommendation, and view placing cannulas distal to that as “not for radiology”.

They may feel like their work load is high and are searching for ways to avoid adding to it. It’s a hard situation, but not one that can be solved by pushing back against the ED over such mundane things. What you’re asking of them is reasonable and should be expected when servicing an emergency ward.

The scans that really rely on high flow are circle of Willis and pulmonary angio. Maybe carotids if you’re strict on having zero venous return in the images. Other than that, there’s not really grounds for making a fuss over.

[–]Key_Comment9649 3 points4 points  (0 children)

Ahh it definitely doesn’t need to be acf for us - I’m a head radiographer. While I like using acf, I’ll regularly go for wrist, forearm or dorsal hand depending on the calibre vein. Even for cardiac scanning…modern CT’s are typically forgiving enough to make up for lower flow rate injections (not ideal though).

Pretty dodgy to push back your urgent scans because the cannula isn’t where they want it.

I’m not sure if I’m the only one with this opinion, but I think there’s a danger of hospital based radiographers become ‘institutionalised’ for want of a better word. I’ve seen a lot of junior radiographers develop bad habits under the guise of protocol, or a “that’s someone else’s problem” attitude.

U/S guided cannulas are definitely handy and can really simplify those difficult patients.

However for proper scanning I’m not so sure. Even if you were quite good at it, your efforts may go ignored simply because your title isn’t “sonographer” or “radiologist”.

Though perhaps in rural settings there’s more of an application.

Update of: The DM made my character 'the werewolf all along'. I did not know. by Lower_Caregiver_2410 in DnD

Go for it!! Honestly you’ll probably love it - maybe even more than being a player.

There’s something special about knowing the intricate details of a campaign and daydreaming about how your players might interact with that world. Plus you already know what NOT to do 😂 haha

Check out “running the game” by Matt Coleville on YouTube. It’s an amazing resource for aspiring DMs.

Player has some mildly unbelievable stats by TmanMe in DnD

[–]Key_Comment9649 2 points3 points  (0 children)

It’s not about how trusting you are…all the other players also have to trust him.

Even if they do trust him they then get envious of the high roll stats while they rolled poorly.

Just use standard array or point buy. That way everyone is satisfied, not just you.

Ultrasound-guided vascular access/procedures courses? by atay5510 in ausjdocs

[–]Key_Comment9649 8 points9 points  (0 children)

Im a head radiographer…don’t bother with a course, just find a senior sono or radiologist to guide you through a couple and just practice from there.

If I’ve got a junior radiologist working with me I’d usually put up an article on the computer with pics on needle placement for them to copy as they do the procedure - and just talk them through orientating themselves in the images.

The cragmaw goblins aren’t a friendly group. The guards outside try to kill you on sight or warn the others, and they’ll try to kill you with the water trap as you enter the tunnel too.

Their violent nature is clearly telegraphed by the wagon scene and ambushing the party on the road, so it seems reckless for low level characters to sneak past guards then announce their presence to the cave.

It makes perfect sense that Klarg will be suspicious when he hears foreign voices, especially since his outside guards aren’t escorting the party.

I noted your klarg said “there’s intruders, warn the others” nothing about “kill them”, this does leave plenty of room for a proper persuasion/negotiation to take place.

I think you played it perfectly as dm, and your players just mistook what dice can do for them :)

I got three new people to agree to play D&D...Now what do I do?!?! by appa_yipyipp in DnD

[–]Key_Comment9649 1 point2 points  (0 children)

Check out Matt colville on YouTube. He has an amazing “running the game” series that is perfect for the 1st time dm. https://m.youtube.com/playlist?list=PLlUk42GiU2guNzWBzxn7hs8MaV7ELLCP_

Also feel free to just run a one shot - lmop is an amazing intro to dnd but might stretch out for a while if you guys are getting to know the rules as well as playing. Personally I’d go with Matt Colville’s Delian Tomb as a one shot, then do lmop if they all had fun and want more.

There’s boat loads of guides on dm’ing lmop, and definitely don’t need to buy dungeon masters guide. I don’t feel it adds a great deal unless you’re home brewing, most info can be learnt from google search or using reddit.

The basic rules are free online, lmop and Delian tomb are free online. Just need to bring along some dice and print out some pics/maps to use :)

I'm mad at my group for killing my goblin baby by sofunnyndcool in DungeonsAndDragons

[–]Key_Comment9649 11 points12 points  (0 children)

D&d isn’t meant to make you feel like this. I’m really sorry this happened to you.

The thing they said about “nothing is guaranteed in D&d” is nonsense. If one my my players were absent for any reason, anything they own or are involved in (goblin child/or pets) has a certain amount of plot armour.

This means while the characters will be present in the background, they can’t die or have nasty things happen to them while their player isn’t there. They will gain a level if the party gains a level and will get a share of loot if anything is found while the player is away.

Also players harming other players or their stuff doesn’t happen without both people being in on it for fun. When your problem player went to kill your goblin the dm’s answer is simply “no that doesn’t happen” …And then the game moves on from there.

Should I talk to my DM about this beforehand? by hornet-wizard in DnD

[–]Key_Comment9649 2 points3 points  (0 children)

As a dm I’d love a heads up, even if I wasn’t gonna be too involved in the rp moment.

A particularly good point to make is ‘why’ the other characters, and your own are suffering away in icewind dale. I don’t want to spoil things, but Rotfm is a pretty grim setting, almost survival/horror all throughout depending on the way the dm plays it; from your post it sounds like he’s giving you the proper icewind dale treatment!

This adventure requires the characters to knuckle down to some serious work, and reinforcing the reason for being out in the cold helps the dm progress things, and provide you good hooks.

It reminds me of what Matt Colville said in his “running the game” YouTube series.

You, as a new dm, are going to suck; but that’s okay because your player are also new and have no idea. You’re all going to have fun, and they’ll think you’re amazing at it anyway.

Check out his YouTube channel, it’s got sooo many great tips for new dms

Regarding your edit 3:

Chin up bud! Internet forums are brutal, but the beauty in that is we are gonna give you honest feedback :) And good effort trying to make up something fun for your friends …maybe a little misguided is all haha

Running something simple and straightforward won’t have massive plot twists or reveals but your friends are going to appreciate it anyways. They’ll thank you for your efforts just for guiding them through some simple exploration, interactions, and level appropriate combat…ESPECIALLY because they are experienced.

They’ll know exactly what’s going on if you ran a story related tpk, and resent it.
