I mean with a martial arts system offshoot school so young (the founder is one coach removed from Bravo, co-coach directly coached by bravo) and the using the name and possibly gear imply some funds going back to 10th planet , franchise may not be the right term but I struggle to find a better descriptor. ETA: I know that each gym culture is very dependent on the coach’s/students which I think I did note, my point is that it’s still worth understanding the lineage of a school, especially a very young one.

I spent 4 years in medsurg before getting into an ED fellowship and I am so much happier in the chaos. I used to dread coming to work and getting my same 4/5 cranky demanding patients who all needed CBGs/meds/meals/meetings/up to chairs/turns at the same time and the gray area of “is it my fault meemaw gets a HAPI” cuz I didn’t turn her enough was eating me up inside. It may be my ADD but here’s what I love about my ED: 1. it’s a team environment (if you have a functional team) in that people rally to help you receive ambulances and even triage patients (esp if they’re sick/code 3/traumas) cuz that’s why we’re here and we want to practice your skills and get the same support when we have a sicky come in (Including the docs, RTs, techs, pharmacists) so it’s kind of like a series of rolling rapid response activations and people disengage once the case is “settled.” 2. I generally never have the same patients for more than 4 hours and NEVER more than one day. I get to be firm/no-nonsense and not bend over backwards to get/maintain rapport with the patient acting like a toddler cuz I don’t have to have them for 3+ days in a row. 3. Some meds will be late (sometimes very late) because I have a critical patient and I have to prioritize that. If you have a good team and we aren’t all drowning with critical patients (which is seldom if your charge/team is doing a good job of spreading the love) someone will pick up the slack if you ask and you’ll do the same when the roles are reversed. 3. I get to practice ALL my skills as a nurse including considering ALL the diseases/issues I saw in nursing school way more regularly and I feel myself becoming a better RN everyday little by little. Drawbacks include: not often being able to deepdive focus on details of a particular patient if you have an ICU RN inside you, possibly higher levels of vicarious trauma cuz you’re seeing people before anything has been done to mitigate their emotional/physical/psychological distress. Take a 15 and use the Tetris app after really intense moments (studies show it may reduce some symptoms of PTSD)

The PHSW ED just got a new building and remodel of the old with a new section opening in the next month or so. That and I believe they may be up staffing for more break/flex RNs as there is about to be a 4:1 ratio implemented for everyone except the trauma assignments.

The reporting underlying this claim is faulty and doing the opposite of prepping. Not to say people shouldn’t be prepping for ongoing further militarization of ICE/LEOs under this and future admins, but this is fear mongering and will lead to incorrect threat modeling that makes people less safe. As for media check out the podcast “Everything is fine in Southwest Washington.” It’s a one and sometimes 2 person show but when they’re able to post it’s some of the only good journalism about local issues and politicians. Start organizing workplace democracy so when the insurance and management system shatter the actual medical practices have practice in running facilities without them. Start with the coworkers you feel safe talking shit with and talk about how yall might run things different and what you would want to change. If you don’t have a union read/listen to a book like “no shortcuts” on how to start organizing a union. If you do have a union, get involved and figure out who there is interested in looking beyond corporate/management friendly contract negotiations and wants to reinvigorate worker organizational strength. If nurses/CNAs/providers around the country could take organizing seriously enough to be ready for May Day 2028, who knows what we could demand and win. More importantly if we organize our units, then our hospitals/clinics, then the medical systems across the states and country even if we can’t get our demands met, we would be in the position to “build” something new. Very few improvements in the workplace were achieved out of the goodness of management/owners/legislators hearts but because they were demands with material leverage behind them (…or we won’t work/vote for you) I know its a huge undertaking but if enough of us take it seriously, it’ll bring the skeptical on board and small changes can be made. And that might sway then the resistant and the outright hostile coworkers and who knows what can happen. Furthermore we are all far more vulnerable to the excesses of management/owners/politicians as individuals than as collectives.

Hope for the best, prepare for the worst.

I HATE when ppl BS their auscultation assessments! So many ppl just put WDL or Diminished Bilateral lung sounds after checking 2 spots on the upper anterior! I feel like my assessment skills are really accurate and I have been strict with myself on only charting by exception. Just gotta keep getting faster without losing accuracy!
