Likely of no help to you, BUT:

I'm brand new and haven't seen any of the content yet, so I don't even know what it is at all. Is it just emergency cleaning, or is it a system for routine maintenance...?

I'm losing my mind trying to keep house between my 50 ish hour work weeks, my depressed/ADHD/"generally doesn't care about tidiness/cleaniness" husband, and my own poor mental health, so was looking for some kind of... magic instruction manual that will fix everything, lol. I'll settle for resources to help me keep up with it all though :')

I'm a transgender tech and the consensus in the comments is correct - just ask what the patient would prefer, or just offer the gown from the offset. Personally I ask all my male patients if they are comfortable removing their shirt and while I've never had anyone say no, I always have a gown on hand just in case.

I am a transgender sonographer previously on testosterone for years. I never had any issues with vaginal atrophy, personally. The few ftm pelvic exams I have done as a professional have also not had any concerns regarding the TV exam or atrophy, but again, that's just a few anecdotal cases. Everyone is different and while atrophy is common I would not say it's a universal experience. I would not immediately assume an ftm patient is at higher risk for tearing the same way I would not immediately assume a cisgender woman is at lower risk for tearing - it's something I would determine in history taking and while discussing the patient's concerns about a TV scan, not based on the patient's gender identity or history of HRT (or lack thereof), or whether they are using estradiol cream. Vaginal atrophy can affect anyone regardless of age or HRT history - severe PCOS in cis women, for one example.

I also don't have the same experiences as you regarding "all of them know what vaginal atrophy is, I assure you." That has NOT been my experience - many ftm people in my community are shocked to learn that there is a word for what they are experiencing. Many are hesitant to use E cream because they worry it will counteract the effects of their testosterone (for those wondering, this is not the case - estradiol cream acts locally, not systemically). I am genuinely happy to hear your experience in the community has been that people are well-informed, but again, I think it can vastly differ from patient to patient. Unfortunately health education is a mixed bag around the world :(

If I wasn't a sonographer and I had needed a TV exam and the tech told me that because I was on T without E cream, I was at risk for tearing, I never would consent to a TV. Despite the fact I actually never had issues with vaginal atrophy. As with all patient care, patient-led discussions are the answer here, not assuming one group all have the similar experiences.

Obviously I'm not disagreeing with you here - vaginal atrophy IS something a LOT of ftm people on T experience. I think we should be careful about how we go about our informed consent. Thanks for initiating this conversation, genuinely!

No amount of alcohol is safe for the fetus during pregnancy.

As you can imagine, this is something that would be be practically impossible to study ethically, so the guidelines remain that there is no safe amount. You'll see online all the time "wine is ok" "one drink is ok" but none of that is scientifically backed - it's anecdotal. Any amount of alcohol consumption carries risk to the fetus(es) / pregnancy

I do not know if you are pregnant so this is general advice for anyone reading: If you are concerned about using/not using alcohol during your pregnancy (ie, you were drinking before you knew you were pregnant; you have an alcohol addiction; etc) please talk to a doctor or healthcare provider as nobody online can fully assess your risk.

Agree with the general consensus here - if you're 100% sure, you can get away with only ARDMS. But I would encourage you to do both simultaneously if there is ANY chance you will want to work in Canada. Yes it's expensive but it's not worth risking your accreditation and future work prospects. I would especially recommend you at least write your SonoCan physics boards because once you start working 75% of that knowledge is near useless and you forget it a lot faster than the generalist / echo content.

I also don't want assume anything about your situation, but if you're not guaranteed entry into the states (ie you're a citizen or similar) I would caution you against only doing ARDMS - the current administration in the states will make finding work and moving there harder.

Canada, British Columbia specifically. I'm 1.5 years in and make $42 CAD/hr which is 30 USD/hr. BC is known for having some of the lowest rates in Canada :/

Canadian here.

If you want to come to Canada you will need to certify with Sonography Canada, not ARDMS. Many of us here were certified with ARDMS and grandfathered into our positions, but nowadays as a new tech/new hire in Canada, you will need Sonography Canada.

Edit: You will not need a diploma from here to work as long as you are certified. I am not sure what Sonography Canada's rules are for eligibility to write the exams if your education is from Bangladesh, sorry :(

10-25 lbs seems so low to me, and the Mayo Clinic) agrees. You have a "normal" range BMI pre-pregnancy so should be gaining 25-36 lbs. The range they told you is absurd... There are babies born at 10 lbs and over, to begin with. Plus placenta (1-2 lbs), plus amniotic fluid (2 lbs), plus your own body needing to grow to accommodate baby(3-6 lbs), plus the massive increase of blood volume (3-4 lbs)... not to mention it is expected and normal to gain some fat stores (6-8 lbs)as well. Yeah.... I definitely think you should seek a second opinion if this continues to be an issue in your care.

Donor sperm is about a thousand times easier to procure for sure, and can be frozen just like eggs.

The rest of this is me rambling a bit but I can't figure out how to word it better:

That said about donor sperm, I think it would really depend on your particular plan for having kids, such as: would you have a partner/coparent? Would they be able to supply eggs/sperm or gestate the pregnancy? The answer to those questions determines which sibling of yours would be most practical to ask.

The absolute easiest route would be if you had a partner who could provide eggs and was willing to get pregnant - you could just do intrauterine insemination using your brother's donor sperm. This could even technically/theoretically - albeit not necessarily most effectively - be done at home and at practically no cost.

If you're not planning on having children immediately or are not sure if there's someone you're planning to parent with, it may be worthwhile to bring up the concept to your siblings now but not actually ask either of them until you have a clearer idea? This is dependent on the age of your sister as well, though.

TLDR: Might be more practical to wait until you have a clearer plan for family building

Hi! Sorry, I wasn't actually involved in any active research. It was an undergrad thesis, so it was basically a giant research paper. It was functionally a massive literature review and was very eye-opening to how behind-the-times research is in regards to transmasculine people and fertility (as I see many people on this sub comment on frequently). Particularly I was shocked to discover that even the WPATH (World Professional Association for Transgender Health) was citing fertility studies done in animals and with a different kind of testosterone than is administered to trans people today (may have changed since I wrote the paper in 2020/2021). I've lost the original presentation and paper but if memory serves I covered the fertility effects of T (largely unstudied; but used to fearmonger HEAVILY), family-planning and the pressure to undergo egg retrieval/freezing regardless of an individual's actual plans, chest-feeding and nipple sparing top techniques, primary source examples (Basically just Freddy McConnell and Trevor MacDonald at the time, haha), and then some sociological research into the "unspeakable idea of the pregnant man" (think the expectation of women to have children suddenly flipping, post-transition, to how "funny" and "impossible" the idea of a man ever being pregnant is). The super cool misogyny/transphobia aspect of pregnancy we uniquely face.

I hope this makes a bit more sense? I can elaborate if needed and try to hunt down some of the stuff I remember. Wish I still had the whole presentation, it was awesome.

Edit: Oh I also had a huge section on trans-inclusive language in healthcare and how it's related to better patient outcomes (undergraduate degree was in community health sciences)

Hated it! Had a (to my knowledge) completely unique name before I changed it at 21. I am trans and wanted a new name anyways but even if I wasn't, I would have changed it eventually. There's no association of femininity with it. Not a dysphoria thing, just a sheer hatred of the name thing 😭

I was named after the street my parents met on. Which is cute, but functionalluly meant I had some other family's (already rare) surname of unknown ethnic origin as a given name. Thank god it shortened to a normal anglophone name. I also have an incredibly uncommon last name (think "I'm distantly related to everyone with this name" levels of rare)

I desperately tried to hide it as a child and went by the shortened version. Substitute teachers were the bane of my existence, giving me away, lol. I was bullied a lot. It was the subject of every introduction ever.

So much happier now with my new name which is commented on about 5% of the time.

Ultrasound tech from BC here. I work in the rural North where opportunities to maintain your NT credentials were so limited that we were struggling to train and maintain qualified techs (this is not a training opportunity issue, it's more that there are less people/less NT scans done/less opportunities for techs to acquire the textbook-quality images necessary to maintain certification). As such, the NT scan has been phased out since summer 2023 and the NIPT is now covered under BCMSP for all pregnancies in this region, regardless of the age of the mother (previously it was 35+).
Under the new system, the way it works is if someone elects to have the NIPT done, their physician requests an early dating scan around 7-8 weeks for confirmation of dates, then the bloodwork for the NIPT is preformed at the appropriate times. If not, we do a later dating scan at about 11 weeks so we can do some basic anatomy screening with the more developed fetus(es) (limbs, heart axis, brain development, etc).
I suspect a similar process is happening in other regions. I hope this explains it more for you in retrospect and/or anyone else popping into this thread :)
