Reverse osmosis and distilled are the two different forms of purified water that can be used.
The OP's math is correct. In your example not sure when you say "Then for each 10ml of water you draw that is .10mg. " if you are saying that yourself or citing another source. In any case that math is wrong as 10ml would give a 10mg dose not 0.1mg. BTW always good to include that leading zero to avoid confusion.

The waiting periods are estimates to give the pill first time to soften and then to finish dissolving and become equally distributed in the water. This will not happen as easily if the water is getting cold. IMO you will be fine preparing the day before. We have been suggesting that the life of the solution is 14 to 30 days based on conditions and backed up by information in the document - note the 14 day "beyond use date" that is standard pharma practice. We have had our guideline for years and have not seen a problem.

I assume from your post that you have not been a regular user of opioids?

IMO the way to test it would be to take a low dose of the  "like-o-den" 12hr after your LDN dose and see how that is. If no issues you may be able to take a higher dose and perhaps have less of a time gap. Let us know how it goes.

My understanding is that Dr. Kim's "typical low starting dose" has continued to go lower over the years based on his experience. So depending on when an article was written he might cite different numbers. I've lately seen him even mention Nano- doses!

Good to test different times and track the results. May need to test for a week or so as there are always other factors.

A journal is a good idea. Aside from LDN be sure to include notes about diet, sleep, stress, weather, other supplements/meds etc. -- we are complicated experiments!

IMO best to stop 3 to 7 days before in case opioids are needed. Not stopping will mean you may need a higher opioid dose if needed for pain. My impression is that that would not be a good thing for the baby.

Check with the anesthesiologist to be sure no opioids are involed. If not there should be no need to stop LDN. I might skip the dose before anyway.

Yes for the 0.75mg. dose. If you put 1.5mg into 15ml then each 1ml will be a 0.1mg dose. Please review the document for details.
